Impingement is when the top of the humerus bone of the upper arm impinges upon one or more of the tendons of the rotator cuff, literally fraying it.

Even if you think youre sitting still, tiny, subtle movements in your position could shift the humerus against the inflamed edges of a rotator cuff tendon. This really hurts.

Another possible cause of sharp pain in the shoulder that comes and goes:

How Is A Frozen Shoulder Diagnosed

If you feel stiffness and pain in your shoulder, see your doctor. A physical exam will help to assess your range of motion. Your doctor will observe as you perform specific movements and measure range of motion of the shoulder, such as touching your opposite shoulder with your hand.

A few tests may also be necessary. Your doctor might do a magnetic resonance imaging scan to rule out a tear in your rotator cuff or other pathology. X-rays may also be taken to check for arthritis or other abnormalities. You may need an arthrogram for the X-ray, which involves injecting dye into your shoulder joint so that the doctor can see its structure.

You can leave a frozen shoulder untreated, but the pain and stiffness can remain for up to three years. A combination of the following can speed up your recovery:

  • physical therapy
  • home care

If You Have To Go To The Doctor

If you talk with your doctor about right shoulder and arm pain, they may recommend the following, depending on the severity of your condition.

  • Immobilization: In some cases, limiting the movement of your shoulder or arm may help it heal. This is often achieved using a sling, brace, or cast. Your doctor will let you know when it can be removed.
  • Corticosteroid injections: This is a type of medication that your doctor can inject into the affected area to help reduce inflammation.
  • Prescription medications: Prescription pain medications may be given for severe pain. Oral corticosteroids can help ease inflammation. If you have RA, your doctor will likely prescribe a disease-modifying drug.
  • Physical therapy: Your doctor may give you a list of exercises to do that can help you regain strength, flexibility, and range of motion. In some cases, they may recommend that you work with a physical therapist.
  • Surgery: Surgery may be recommended when other treatments havent been effective or when damage is severe. The specific procedure thats used can depend on whats causing your condition.

Things You Can Do If You Have Shoulder Impingement

  • Avoid things that make the pain worse avoid activities that involve repeatedly lifting your arm above your head for a few days or weeks. Ask a GP or physiotherapist when you can restart these activities.
  • Do not stop moving your arm completely try to carry on with your normal daily activities as much as possible so your shoulder does not become weak or stiff. It’s usually best to avoid using a sling.
  • Gently hold an ice pack wrapped in a towel on your shoulder for 15 to 20 minutes a few times a day.
  • Take painkillers anti-inflammatory painkillers or paracetamol may help. A GP can prescribe stronger painkillers if needed.

What Are The Symptoms Of A Shoulder Injury

Rotator Cuff Tears and Shoulder Pain at Night

The shoulder’s ball-and-socket joint gives you great range of motion, but it comes at the expense of stability. The shoulder joint gets dislocated more often than any other joint in the body. And repeated stress from the way you use your shoulders on the job or playing sports can lead to tears and other injuries.

Ask yourself some questions to help you decide if you have a shoulder injury:

  • Can you move your arm normally, or is your shoulder too stiff or painful?
  • Do you feel like your shoulder could pop out of the socket?
  • Is your shoulder strong enough for things you normally do?

You can treat some shoulder injuries at home for a few days with rest and ice. You can bandage it to hold it in place if necessary, and elevate it above the heart. But some injuries need professional help. Here are signs that you need to see a doctor right away:

  • Your shoulder joint looks deformed.
  • You canât use your shoulder at all.
  • The pain is intense.
  • The shoulder is swelling suddenly.
  • Your arm or hand is weak or numb.

Some Shoulder Conditions May Become More Common As You Age

You probably don’t think about your shoulders much, until you suddenly experience pain in one of them. Shoulder pain can make a simple act brushing and drying your hair, reaching behind your back to fasten a bra, or grabbing something overhead seem like a monumental task.

As you age, you’re more likely to experience shoulder pain from a variety of common conditions. “Shoulder problems are very common,” says Dr. Arun Ramappa, associate professor of orthopedic surgery at Harvard Medical School. The pain can come on gradually or abruptly, and it may range from mild to excruciating.

Below are some of the most common conditions you may encounter, and some tips for how to address them.

Pain Around The Upper Arm And Shoulder

This happens when the pain is localized predominantly in the area of your shoulder and upper part of your humerus. The conditions that cause this can include:

  • Rotator cuff disease: Initially, pain is often felt at the front of the shoulder.
  • Torn rotator cuff: Tears in the rotator cuff can include pain that begins near the shoulder.
  • Dislocated shoulder: Pain from a shoulder dislocation is most acutely felt near the shoulder.
  • Broken collarbone: The pain from a broken collarbone can be felt close to the shoulder, particularly when you try to move your arm.
  • Frozen shoulder: This type of pain typically comes on gradually and feels dull and deep.
  • Calcific tendinitis: Pain due to calcific tendinitis can be severe and can come on suddenly, often in the morning.
  • Shoulder sprain: You can feel pain from a shoulder sprain not only in your shoulder, but also in the area of your upper arm and collarbone.
  • The pain due to a separated shoulder is felt around the shoulder and upper arm.
  • Shoulder osteoarthritis and RA: People with arthritis in their shoulder often feel a deep ache in their shoulder and upper arm.
  • Brachial neuritis: Pain from brachial neuritis is typically severe and most often happens in the area of the shoulder and upper arm.
  • Brachial plexus injury: Pain from a brachial plexus injury can happen around the shoulder and upper arm. It may be long lasting.

Why Does This Occur At Night

There are several reasons why shoulder pain at night occurs or is aggravated. The common explanations include:

  • Sleep typically involves a static position lasting hours at a time with little or no movement.
  • Sleeping on your side places additional pressure on the tendons and bursa of the shoulder
  • Sleep can cause muscles and tendons to settle in a slightly different position resulting in additional pressure and reduced blood flow.

    If one shoulder hurts when sleeping, try laying on the other side. While people often do not control their bodies while asleep, you could try building a wall with pillows to prevent rolling over onto the injured shoulder while asleep. If that doesnt help, try sleeping semi-upright either in a chair or by propping a few pillows under the upper body to alleviate pressure on the affected shoulder. Ask your doctor for a list of stretches that can be done to loosen the shoulder these may be especially beneficial to do before going to bed.

    However, if shoulder pain is a fact of life for you, it may be time to seek treatment.

    Your orthopedic doctor will physically examine your shoulder, assess your medical history, and may order tests that can help with a diagnosis and identifying the cause of your shoulder pain. In general, an X-ray can identify structural issues such as bone fractures or spurs or the presence of arthritis for soft tissue assessment, an MRI may be required.

    Causes Of Shoulder Impingement

    When you lift your arm, the rotator cuff tendon passes through a narrow space at the top of your shoulder, known as the subacromial space.

    Shoulder impingement happens when the tendon rubs or catches on the bone at the top of this space, called the acromion.

    This can be caused by:

    • the tendon becoming swollen, thickened or torn this can be due to an injury, overuse of the shoulder or “wear and tear” with age
    • the fluid-filled sac found between the tendon and acromion becoming irritated and inflamed this can also be caused by an injury or overuse of the shoulder
    • the acromion being curved or hooked, rather than flat this tends to be something you’re born with
    • bony growths on the acromion these can develop as you get older

    Why Did I Wake Up With Shoulder Pain

    If you have no history of shoulder problems the most likely answer has to do with your sleep position. People who sleep on their side, frequently sleep on their shoulder in a way that causes their rotator cuff tendons or shoulder bursae to become pinched. It may not hurt right away, but after several hours this position creates an inflammatory response. It is this inflammation of the pinched structure that is so painful when you wake up.

    Rest assured that for most people, symptoms will decrease within the first 48 hours. But some will still be left with persistent shoulder pain. If you are one of those unlucky people, a consultation with an experienced Physiotherapist is in order.

    Rotator Cuff Injuries And Irritations

    Your rotator cuff is a group of muscles and tendons that help tether your shoulder into the socket and allow you to move it in a circular motion. Some two million people visit a doctor each year for rotator cuffrelated issues, according to the American Academy of Orthopedic Surgery . Most problems with the rotator cuff fall into two categories: tears or inflammation. Suspect a rotator cuff problem if you have pain or stiffness in your shoulder when you lift your arm above your head to brush your hair or when you reach behind your back. “You won’t be able to do a lot of things you want to do, such as put dishes in an upper cabinet, play tennis, or do garden pruning,” says Dr. Ramappa.

    Rotator cuff impingement. A rotator cuff impingement happens when there is irritation, inflammation, or compression of the tendons or bursa in the shoulder. An impingement can be caused by an injury, but it can also just result from general wear and tear from daily life.

    Rotator cuff tears. A tear in the rotator cuff will produce pain that is similar to an impingement but has one additional differentiating feature. “If the pain is associated with weakness, it is likely caused by a tear, and if you just have pain, it may only be a rotator cuff impingement,” says Dr. Ramappa.

    Performing simple stretches two to three days a week can help keep your muscles flexible and reduce pain, provided your doctor approves.

    It’s time to seek medical attention if you have new, worsening, or severe shoulder pain. Other symptoms ought to spark the same reaction:

    • Abdominal pain
    • An injury that causes joint deformity
    • Inability to carry objects or use your arm

    • Shoulder pain that occurs at night or while resting
    • Shoulder pain that persists beyond a few days

    Symptoms Of Collarbone Pain

    Believe it or not, the collarbone is the most commonly broken bone in the body. Also known as the clavicle, the collarbone is the relatively straight bone that runs horizontally from the top of your sternum to your shoulder. It provides stability and protection for the delicate structures underneath. Many of the muscles that allow you to move your arm attach to bony structures around the collarbone, and nerves and blood vessels run deep around the bone as well. Pain in this region can be from an injury of the collarbone itself or from irritation of the structures that surround it. However, there are some conditions that may mimic shoulder pain but are due to a different, serious issue.

    Front Shoulder Pain And The Rotator Cuff

    There are a number of different ways that the rotator cuff can become compromised. One common issue is impingement. If you raise your arm to shoulder height or higher, the gap between the rotator cuff and acromion narrows. Sometimes the acromion will rub against the tendon and cause it to become irritated and painful.

    Two other common issues with the rotator cuff are bursitis and tendinitis.

    The bursa is a sac filled with lubricating fluid, located between tissues such as bone, muscle, tendons, and skin, that decreases rubbing, friction, and irritation. If the bursa becomes inflamed or irritated it is referred to as bursitis. It is often caused by overuse or repetitive movement in the shoulder.

    This can obviously happen in sports like tennis where People practice the overhead serve on a repetitive basis. But the condition can also be caused by other activities such as painting a ceiling. Even poor posture can lead to bursitis.

    Tendinitis occurs when the tendons of the rotator cuff become inflamed.

    What Are Shoulder Pain Treatments And Home Remedies

    For dislocations, separations, and fractures, you need a doctorâs help to get your shoulder back in the right position and then a sling to hold it in place while it heals.

    For many other issues, your doctor may suggest rest, heat or ice, and a medicine like aspirin or ibuprofen to reduce the pain and swelling.

    If your shoulder doesnât improve after these first steps, your doctor may try injecting a corticosteroid straight into the joint to relieve swelling and pain.

    Sometimes cartilage tears, rotator cuff tears, and frozen shoulder donât improve with rest and medicine. Your doctor may recommend surgery.

    With any problem in your shoulder, your treatment plan will probably include exercises to help you stretch and strengthen the joint, and to improve your range of motion.

    Bursitis happens when the bursae in the shoulder become swollen and inflamed. Bursae are small sacs fluid that make it easier for the tendons to glide over the bones when you move. Strain and overuse can cause inflammation in these sacs. This can make it painful to use your shoulder even for simple tasks.
