Posterior Cruciate Ligament Injuries
The posterior cruciate ligament plays a similar role to the ACL, though it is less likely to become injured than the ACL.
PCL injuries may happen during traumatic events, such as falling directly onto the knee from a height or being in a vehicle accident. With enough force, the ligament may tear completely.
PCL injuries cause symptoms such as:
- knee pain
- stiffness in the knee if bending
- difficulty walking
- swelling in the knee
Completely resting the knee may help a PCL strain heal. However, a severe PCL injury may require surgery.
When Do You Feel Better Or Worse
For instance, does walking up or down a flight of stairs trigger pain behind your kneecap? That could be a sign of osteoarthritis. With osteoarthritis, pain also tends to get worse over the day as youre more active.
On the other hand, pain that starts strong in the morning and gets better as you move during the day sounds more like an inflammatory condition, such as rheumatoid arthritis.

Knee Cysts And Bursal Injury
Injury to bursae is usually the result repetitive motions and will elicit pain and tenderness. Cystic lesions of the knee can be caused from a diverse group of entities from benign etiologies to complications of arthritis, infection, and malignancy. The classic cystic lesion causing posterior knee pain is the Bakers cyst. A Bakers cyst is caused either by a herniation of the synovial membrane through the posterior capsule or by an escape of fluid through an anatomic bursa next to semimembranosus or gastrocnemius. Fluid seeps in to the popliteal bursa, located at the back of the knee causing it to swell. It often feels like a squashy orange. A Bakers Cyst typically causes pain behind the knee when bending the knee as the bursa gets squashed.
There is minimal literature available on neurological causes of posterior knee pain as there are few case reports, randomized control trials, or meta-analyses that discuss the neurological causes of posterior knee pain. However, referred pain has been implicated as a cause of posterior knee pain. The patellofemoral joint and lumbar spine may both refer pain to the posterior knee. Pain can also be caused by entrapment of nerves in the popliteal fossa.

Pain Behind The Knee Causes: Popliteal Injury
Do you feel discomfort in the knee area when walking, pain behind the knee when bending and stretching or just a general burning sensation in this area? It is possible you are suffering from a popliteal injury. This can also be recognized if, when palpating, there is a muscular bulge or contracture in the subsidence of the area.
The popliteus is a muscle that is located just above the calf and behind the knee. It is a very small and short muscle and not many people know it exists until injured. However, this small muscle, sometimes forgotten, has its function. For example, when the knee is extended, the popliteus is responsible for initiating the flexion. In addition, it maintains balance of the knee allowing for stability.
If you go running regularly you will know that this exercise type can impact the legs greatly. This muscle also intervenes in this striding movement. The impact receives by constantly hitting the ground can cause the popliteal muscle to overload, which can cause stiffness and lack of elasticity in the knees. This can then cause the tendon to swell and result in tendinitis. Popliteal tendonitis can result as pain in the knee, specifically in the back of the knee. Do you feel pain behind the knee after exercise, especially after or while doing squats or running downhill? Then it is possible that you are suffering from this injury.

May be due to chondromalacia patella. In this condition the cartilage on the underside of the patella softens and deteriorates. Some people are able to ignore the condition, but it will not improve and will probably need surgery. Sometimes an unstable flap of surface cartilage may cause this pain, and may be curable with simple keyhole surgery and a chondroplasty.

Conditions Due To Cartilage Damage
There are two triangular, or wedge shaped, pieces of cartilage in the synovial joint which are called menisci. One lies on the inside, or medial side, of the knee, and one on the outside or lateral side. They act as shock absorbers between the femur and tibia, the end surfaces of which also have a cartilage covering called articular cartilage. At the front of the synovial joint is the patella which is an oval bone protecting the front of the joint and lined internally with cartilage.
Posterior Knee Pain Exercises
Pain behind the knee can be quite debilitating, as we use our legs to walk every day. If you are unfortunate enough to suffer from knee pain, certain exercises may help.
Prone wall stretches While standing, lay your back flat on the surface of a wall. Place the heel of your injured knee on to the wall without bending the knee. Increase the stretch by moving your body closer to the wall and hold for 30 seconds. Then slowly move your body away from the wall, decreasing the stretch. Repeat this exercise about eight times.
Leg to chest maneuver While sitting upright in a firm chair, place both feet firmly on the floor. Now, slowly lift the leg with the contracted muscles off the floor and bend your knee as you lift your leg toward your chest. You can also place your hand behind your knee to increase the stretch. Hold this position for five seconds. This exercise will help to loosen your contracted hamstrings.
Upright wall stretch Stand about three feet from the wall as you open your palm and place them on to the wall at shoulder level. Now slowly lean forward, bending at your elbows and keeping the rest of your body straight. It is also important to keep your feet flat on the ground while doing this to stretch the calf muscles.

Whats Behind The Knee
The back of the knee has complicated anatomy.
As well as the knee joint that sits in the middle, there are muscles from the thigh and calf that pass through this area. Firstly, the large hamstring muscles start from the pelvis and pass across the knee to attach to the lower leg bones. In addition, the large calf muscles start at the bottom part of the thigh and cross the knee to form the large Achilles tendon.
Finally, there are important nerves and blood vessels that cross the back of the knee to supply the lower leg and foot.
Pain Behind The Knee: Self
Pain in the hollow of the knee and behind the knee, respectively, is mostly caused by tensed muscles and trigger points in areas of the calf, knee and the back of the thigh.
Fortunately, serious injuries are rarely the cause, especially if there is no known trauma or injury.
Even when there is structural wear and tear, such as damaged cartilage etc., knee pain can often be alleviated by getting rid of excessive muscle tension and sometimes even eliminated.

What Causes Pain Behind The Knee When Bending
The most common cause of pain behind the knee when bending is a Bakers Cyst. This is when there is inflammation of the semimembranosus bursa, a small sac filled with fluid that sits at the back of the knee.
If the bursa gets inflamed, then any time you bend your knee, the bursa gets squashed, causing posterior knee pain.
Pain Behind The Knee When Bending It Or Pain And Stiffness Below The Kneecap
Although not behind the knee, the patellar tendon can be overloaded and torn by repetitive actions such as jumping, running or kicking. This is called patellar tendonitis, and can get progressively worse as the the torn tendon swells and becomes weaker. Again the RICE formula should be applied, and your doctor should be consulted.

Massaging The Areas On The Side Of Your Calf: Gastrocnemius
For this massage, I recommend you use the thumb technique.
Sit down on a chair with your foot flat on the floor and press into the muscles on the side of the calf the inner side as well as the outer and search for painful areas.
As soon as you find one, massage over it no more than 15 times from the bottom upward.

Tennis Leg Causing Muscle Pain And Swelling Behind The Knee
A condition called tennis leg can also cause pain in the back of your leg behind the knee.
Tennis leg is associated with damage to the gastrocnemius where damage to the muscle and the soleus muscle in the calf causes a buildup of fluid.

An injury to your hamstring is a very common reason for sharp pains at the back of your knee.
Your hamstring starts at the bottom of your pelvic bone and crosses the back of the knee and ends at the lower leg. Doctors from the American Academy of Orthopedic Surgeons say that your hamstring helps you bend your knee.
Hamstring injuries can result in pulling the hamstring muscle or completely tearing it. This can result in pain at the back of your leg below your knee. You may also notice bruising of your leg and may find it difficult to walk for a few weeks.3

What is the cause of the pain?
Is it Arthritis, cramp or a tear in the meniscus.

A Sudden Severe Pain Or Popping Sound Accompanied By Swelling And A Feeling Of Looseness Of The Joint And Instability
This may be due to damage to one of the knee ligaments, and is commonly seen as a result of sports injuries concerned with a sudden impact, twisting or over stretching. If the ligament is torn or twisted it is a sprain . If you suspect a damaged ligament you should comply with the RICE procedure, but not immobilise the joint as there is a risk of stiffness and possible muscle atrophy. The injury should be clinically assessed, and may need surgery.

But What Caused All Of These Issues In The First Place
With the vast majority of acute & chronic issues, its worth playing devils advocate & really diving deep into asking: what actually caused the issue in the first place?
