What Causes Knee Pain Without Injury
Sometimes the cause of your knee pain is obvious: a twist, misstep or strain can strike at any time during a walk or a run. Other times, patients can suffer knee pain without injury and are unsure of the source of their discomfort.
Youll need to consult with a physician to determine the cause of your knee pain, but there are several common causes of knee pain without injury.
What Causes Pain On The Inside Of The Knee
MCL Injury – The most common cause of inside knee pain is an injury to the medial collateral ligament . The MCL is the ligament runs up the inside of the knee joint and it helps stabilize the inside of the knee. MCL injuries typically occur when impact happens on the outside of the knee, which overstretches or tears the inside ligament.
Meniscus Injury – The meniscus is the piece of cartilage that cushions the knee joint as we walk. Depending on where on the meniscus an injury has occurred, a patient may experience inner knee pain.
Other Causes of Inside Knee Pain – Weakness of the muscles around the knee or arthritis may also cause pain on the inside of the knee. There may also be other causes for inside knee pain, but the ones listed here are the most common.
Pain On The Outside Of The Knee
- A burning pain at the outside of the knee may be due to iliotibial band syndrome. The iliotibial band is a ligament running down the outside of the thigh to the outside of the knee which can become inflamed and irritated.
- A tear in one of the two menisci can cause pain, swelling, and a feeling that the knee is giving way or locking.
- A burning sensation at the side of the knee can indicate pressure on the menisci and sometimes can be due to a fluid filled cyst.

What Causes Pain Inside Of Knee
Your knee is one of the most complex joints in your body, with many interworking parts. Bones, ligaments, cartilage, tendons, and menisci must all work together in perfect alignment to keep you moving fluidly. When one of these components becomes strained, torn, or injured, it throws the entire system off base, resulting in discomfort and difficulty in moving.
Inner knee pain is often caused by cartilage deterioration or a sports injury, but there are several other possible culprits as well. Pain inside the knee can be caused by:
- Osteoarthritis: This degenerative disease breaks down cartilage to the point where bones end up grinding against each other. Without the protective cushioning cartilage provides, this can create serious pressure in your inner knee.
- Rheumatoid arthritis: If you experience inside knee pain in the morning that subsides throughout the day, you may have rheumatoid arthritis, which causes joint inflammation.
- MCL injury: Youve probably heard of athletes tearing their MCLthis happens when the ligament that runs alongside your outer knee over stretches.
- Knee contusion: Landing hard on your knee or getting hit in the same area can bruise your knee bone and cause pain inside your knee.
Common Causes Of Inner Knee Pain
There are a variety of causes of inner knee pain. Many of them can be linked to an injury. Some of the most common incidents that cause knee injury and pain include falls, sports injuries, or increased activity.
Adults particularly those older than 60 are most likely to experience knee pain. However, inner knee pain can also occur in children and adolescents.

Anterior Knee Pain Causes In Young Athletes:
Before you read this, it is worth repeating the majority of you will not have the issues that I outline below. The majority of you will have pain due to a gait abnormality or muscle imbalance. We all feel the need to have a specific label for why our knee hurts. Anterior knee pain due to muscle imbalance just doesnt cut if for some of you you want something more specific. I understand that.
The point I am trying to make is, if you assign the cause of your pain to a specific structural finding or something that your MRI report mentioned, then you may have difficulty getting past that and buying into the PT or exercise strategies that have been shown to work.
- Chondromalacia: Chondromalacia is likely a very early sign of arthritis. It involves the softening of the cartilage under your patella . As the cartilage softens, it is thought that it irritates other tissue in the knee with certain activities. Im not convinced that this is a common cause of anterior knee pain. Chondromalacia is very common, even in people with no pain.
We find chondromalacia in so many people at the time of surgery for other things, such as an ACL tear. And those people rarely have pain in the front of their knee. So it is not entirely clear why some people might have pain due to chondromalacia, and others do not. Surgery is rarely needed.
Why Does My Knee Hurt When I Straighten It
Your knee is made up of bones, cartilage, muscles and tendons. When you extend your knee, your quadriceps muscles tighten, and your hamstrings relax. Pain on knee straightening is usually indicative of damage or overuse of the quadriceps muscles, leading to tiny tears in its tendon. Pain may also occur due to any damage to the joint itself. You may get pain specifically in the back of the knee due to cyst formation following injury of the joint.

What Is Causing My Inner Knee Pain
Let’s have a look at the common symptoms associated with medial knee pain and what they typically indicate:
Inner Knee Pain When Straightening Leg:Pes Anserine Bursitis is the main culprit here as the bursa can easily get squashed when straightening the knee.
Medial Knee Pain With Flexion: Most medial knee pain gets worse with knee flexion, especially when weight bearing through the leg. If it’s worse when standing, it may indicate an MCL tear or meniscus tear. If it happens when you are sitting or bending the knee, it may be pes anserine bursitis or plica syndrome.
Anterior Medial Knee Pain: If your inner knee pain is coming round to the front of the knee, it may actually be a problem with the knee cap or Runners Knee rather than one of the structures on the inner knee.
Medial Knee Pain When Sitting Cross Legged: Pain in the inner side of knee when sitting cross legged is most likely due to a meniscus tear as this position places extra stress on the cartilage.
Osteonecrosis Of The Knee
Osteonecrosis is a condition in which bone tissue dies due to a lack of adequate blood supply. People typically notice severe pain on the inside of the knee with tenderness and joint swelling as well as pain when bending or straightening the knee. These symptoms may cause a person to limp when walking.
Osteonecrosis in the knee is not common. It is most likely to occur after an injury, but it can also develop gradually in absence of a trauma. Older women who have osteoporosis are at the greatest risk for osteonecrosis.

Medial Collateral Ligament Tear Or Sprain
The MCL is a band of tissue that runs along the inner edge of your knee. It plays a role in connecting your shin and thigh bones to keep your knee stable and working properly. The MCL can tear when it stretches far enough or if the outside of your knee is hit very hard. When your MCL is injured, your knee has the capability to over-extend itself, or bend too far in a direction its not supposed to. That is why its important to get treatment. Varying types of pain
The pain can be sharp, dull, or burning. It may come on suddenly or gradually. It may be constant, or it may occur when you put weight on the leg or when you bend the knee. This information can help a doctor diagnose your knee problem.
Swelling or stiffness
The knee may look swollen or misshapen. You may be unable to bend the knee, or your knee may pop, lock up, or collapse when you put weight on it. These symptoms usually indicate that you have sustained an injury, but there are other possibilities as well.

How Can You Tell The Difference Between A Torn Mcl And Meniscus
A medial meniscal tear can be mistaken for an MCL sprain because the tear causes joint tenderness like the sprain. With a valgus laxity examination, a medial meniscal tear can be differentiated from a grade II or III MCL sprain. The presence of an opening on the joint line means the medial meniscus is torn.
Inside Knee Pain After Running
What do we mean when we refer to the “inside” or “inner” knee? Good question. There are a few different parts of your knee that can be injured or irritated from exercise, particularly running.
The inside knee is the relatively small area on the inside of the leg closest to your other knee. So for your left knee, this area would be to the right of the kneecap.
Others runners that have experienced this pain refer to it as a “twinge”, “nagging pain”, or “ongoing soreness” rather than a sharp or shooting pain. But everyone experiences muscle and joint pain differently. If any of these terms accurately describe how the inside of your knee feels, keep reading.

What Treatments Shouldnt Be Offered
There are many other treatments that have been used for osteoarthritis, including needle lavage for joints, lateral wedge insoles, acupuncture, glucosamine, chondroitin, intra-articular injections of hyaluronic acid, and arthroscopy. However, studies of these treatments have not consistently shown statistically significant or clinically important improvements, and their use is not routinely recommended in guidelines.
Needle lavage for joints was assessed in a 2010 Cochrane review, and the authors found no benefit in terms of pain relief or improvement in physical function. Neither the EULAR nor the AAOS guidelines recommend using lateral wedge insoles because of lack of efficacy in comparison to neutral insoles, and reports of adverse events. A recent meta-analysis investigating the use of lateral wedge insoles for treatment of medial knee osteoarthritis failed to show a statistically significant or clinically important improvement in pain scores using the Western Ontario and McMaster Universities Osteoarthritis Index when compared with a neutral insole. The Canadian Orthopaedic Association, as part of the Choosing Wisely Canada campaign, does not recommend the use of either joint lavage or lateral wedge insoles for the treatment of knee arthritis .
The Problem No One Is Talking About
When youve had your pain more than 2 months and youve seen 3-5 other doctors without more than 50% relief, you and I can probably both agree that theres something else going on.
The pain on the inside of your knee can be caused by failed ankle or hip tests.
You would fail any of the following:
- Knee to Wall Test with your toes 6 away from the wall, can your knee touch it with your heel down?
- Thigh to Chest Test lying on your back, can your thigh touch your ribs?
- Lunge Stretch Test kneeling, can the front of your hip move 12 in front of your knee?
If you failed any of those tests, your knee would start as inflamed and get better with rest. As time went on and you didnt fix the failed test, youd be risking the meniscus, MCL, or arthritic damage you currently dont have.
In order to get the failed test fixed and passing so that your knee operates at 100%, you need to find a adhesion specialist to find out if you have adhesion for any muscles that are responsible for stopping the joint in the correlated test.
If all of those tests are passing, the adhesion specialist would look at the knee joint itself.

For many, the culprit behind that nagging soreness is osteoarthritis, which affects an estimated 31 million Americans quite often, in this particular joint according to the Arthritis Association.
But there are other common causes of knee pain, stemming from the fact that knees are our largest, most complex joints. Knee joints allow you to stand up straight, walk stairs and get up and down from sitting, says Daniel Saris, an orthopedic surgeon at the Mayo Clinic and professor of orthopedic surgery at the Mayo Medical School. But they are also the most difficult joint because they’re not stable. Hip joints and ankles are both pretty stable by themselves, but the knee is just three bones trying to be good friends, and they need muscles and ligaments for stability.”
Knee pain is not to be taken lightly. A Japanese study, published last year in the Journal of the American Geriatrics Society, found a link between the onset of knee pain in people 65 or older and depression. And a recent study from the University of North Carolina School of Medicine Thurston Arthritis Research Center and Harvard’s Brigham and Women’s Hospital found that knee pain in men and women over age 45 correlated with higher rates of death.
Treatment Options For Inner Knee Pain
The first step in treating inner knee pain involves at-home remedies, such as resting and icing the joint for a few days. You can also try over-the-counter anti-inflammatory medications such as aspirin or ibuprofen.
If the pain persists, an orthopedic specialist may prescribe corticosteroid injections to lessen your discomfort. Only in the most severe cases will a doctor recommend surgery. For example, minimally invasive arthroscopic surgery may be required to repair or reattach the ligament in the case of an MCL tear.
Physical therapy exercises to strengthen the muscles encasing the knee are another common treatment option. A quad stretch, for example, helps reduce stress on the knee. For this exercise, use a chair for balance and support. Then, standing with your feet shoulder-width apart, step back with your right foot, maintaining a straight posture. Bend your knees slightly so you feel a stretch in the front of your legs. Hold the stretch for 10 seconds and then repeat with the other leg.
