The root cause of sciatic pain needs to be diagnosed by a professional, such as a chiropractor. Some of the more common causes of sciatic pain include:
- Lumbar disc herniation
- Isthmic spondylolisthesis
- Spinal tumor
There are other possible causes, but those are rare. Before your sciatic pain can be relieved, the root cause needs to be determined and addressed.

The first step is to take painkillers to control the pain and make yourself more mobile. You can combine paracetamol and ibuprofen to relieve pain and reduce inflammation.

Note: Piriformis Syndrome Feels Like Sciatica But It Isnt
There is a muscle where the thigh bone meets the buttocks called the piriformis. When this muscle becomes tight, it can irritate the sciatic nerve, causing similar pain, but this isnt truly sciatica, where the nerve is compressed.
Why does this matter? The treatments for piriformis syndrome and sciatica are different. Your chiropractor will be able to diagnose what the root cause is and whether or not you have sciatica.
Treating Sciatica Pain And Managing Expectations
Many people think that the worse the pain, the more likely something bad is going on. However, this isnt true for sciatica. The body can reabsorb the disc material that is causing symptoms, even for those with severe pain. So, treatment focuses on controlling pain and keeping people as active as possible. If the pain is excruciating, lying down for short periods can help, but prolonged bed rest does not. So, once the pain diminishes, I tell patients to get up and start walking short distances. Patients often ask about spinal injections where steroid medicine is injected into the affected area. It is worth considering for those with uncontrolled pain or for those with persistent, bothersome symptoms who want to avoid surgery. Injections can provide short-term relief. Like any procedure, it has uncommon risks including more pain, and it doesnt seem to decrease the need for future surgery.

No Pain No Gain Mindset
This is a great way to build mental toughness, but a TERRIBLE way to rehabilitate your lower back. You need to learn how to pace yourself appropriately when returning from a back injury. You need to think of yourself like a marathon runner rather than a sprinter. If you come off the starting line at a 4-minute mile pace you are going to crash and burn before you know it.
Healing and pain are NOT the same thing. Asking, how long will it take to heal? is a totally different question than how long will it take for the pain to go away? You can be Pain Free with a back that is still healing if you learn how to pace appropriately. Avoiding painful postures can help desensitize angry nerves. Allowing you to do more activity with less pain over time.
Words of Wisdom: Understanding that hurt does does equal harm is just as important. A little bit of pain here and there is ok, but keep it to a minimum. If you can only tolerate walking, shopping, house work, etc. for a short period of time you need to do shorter, more frequent bouts of activity. This strategy over time will lead to steady progress without set backs.

Medications For Relief Of Pain From Sciatica
Several types of medications may be used for sciatic pain. Oral medications include:
- Over-the-counter pain relievers such as acetaminophen, aspirin, or NSAIDs
- Prescription muscle relaxants to ease muscle spasms
- Antidepressants for chronic low back pain
- Prescription pain medications for more severe pain
Do not give aspirin to a child aged 18 years of age or younger because of the increased risk of Reye’s syndrome.
In some cases, a steroid medication is injected into the space around the spinal nerve. Research suggests these injections have a modest effect when irritation is caused by pressure from a herniated, or ruptured, disc.
Sciatic Pain Treatment Options
Treatment of sciatic nerve pain usually includes a combination of over-the-counter medications and home care. For about 80 to 90 percent of those with acute sciatica, symptoms resolve themselves within several weeks, according to the American Academy of Orthopaedic Surgeons .
Chronic sciatica is a life-long condition, and doctors might suggest more aggressive treatment. Although the pain with chronic sciatica is usually milder, it might require treatment during flare-ups. There are several treatment options, including home care, medications, corticosteroid injections, physical therapy, and surgery.
