Many people learn as kids that an ice pack is great for reducing pain and swelling. But, if ice is left on the skin for too long, it can cause damage. So, here are some things you can do to safely use ice on your pinched nerve.

Patients always ask me about the best carpal tunnel remedies I know. After decades in this field, I’ve heard about hundreds of “home remedies” or “quick fixes”. Some of these are reasonable. But many others are… well… bizarre.
If you have carpal tunnel symptoms, I’ve outlined some of the most common treatments patients have tried. They’re in no particular order.Some are great and some simply don’t work at all.
Carpal Tunnel Pain Relief At Night
One of the big problems with carpal tunnel is that people often unconsciously bend their wrists when they sleep, exacerbating carpal tunnel. This can be hard to remedy since we dont have much control over what we do when were asleep. However, there are some ways to get relief from CTS at night.
- Shake out your wrist just before bed.
- Try alternating hot and cold therapy before bed.
- Do 5 to 10 minutes of carpal tunnel exercises just before bed.
- Wear a wrist brace while you sleep.
However, like ice and heat therapy, these only address the symptoms of CTS. For proper treatment, its best to see a chiropractor or physical therapist.

How Do You Know If Your Carpal Tunnel Is Severe
When you have developed carpal tunnel syndrome, youll have some discomfort from time to time that you learn to live with but can go on about your day. Some days, your hands are weaker than others. You may drop things more frequently. As it becomes more severe, though, you may need to have a doctor examine you and see if any damage has happened, that may indicate you need Carpal tunnel treatment.
Over time, the muscles could shrink in your hand, theyll begin cramping more, and the pain will become worse. The function of the median nerve starts to slow down from the irritation and pressure around it. Physical Therapy And Exercise For Carpal Tunnel Syndrome
Research shows that physical therapy can be an effective treatment for carpal tunnel syndrome. Your PT may use heat and ice with compression to help control the inflammation. Other anti-inflammatory treatments, like ultrasound or iontophoresis, may be used as well.
The most effective treatment in physical therapy for CTS is exercise. Exercises typically include nerve gliding techniques to keep your median nerve mobile while it is healing. Studies show that performing nerve and tendon gliding exercises can improve wrist and hand mobility while helping to improve hand function and decreasing pain.
Sometimes, physical therapists use paraffin baths to help improve mobility and circulation to your median nerve. Paraffin is a wax that is applied to your wrist and hand by dipping your hand into a wax bath. The wax heats your wrist and hand, providing much needed circulation and mobility to the tissues around the tendons and nerve in the carpal tunnel.
Your physical therapist can also help determine if a specific activity is a possible cause of your carpal tunnel syndrome. He or she may ask you to adjust your workspace to limit stress and strain on your wrists. Specialized wrist supports for your keyboard or mouse pad may be used to keep your wrists in a neutral position while working to help lessen stress on your median nerve.

Is It Okay To Put Heat On A Pinched Nerve
Overall, its okay to put heat on a pinched nerve. However, there are times when ice is best, and other times when heat is most beneficial.
The general rule of thumb for a pinched nerve is to use heat only after youve managed to get the pain to subside a little bit. When the pain first flares up, use ice or cold packs before you use heat. And dont apply heat to the area directly after applying ice. Youll want to wait 30 minutes to an hour at least.
For the best results, keep the heat on the affected area for 10 to 20 minutes at a time. Take a minimum 30-minute break between sessions. If you find that its helping, you can apply moderate heat to an area for up to an hour or more. Extended heat therapy is most beneficial for more severe pain from a pinched nerve. Its almost like spending time in a hot tub you can do it safely if its not too hot.

When Your Sciatica Warrants A Visit To Your Doctor
Its important to recognize when at-home therapies arent easing your sciatica. If these treatments dont help you, it may be time to see your personal doctor or spine specialist.
People avoid the doctor for a variety of reasons. Maybe youre unsure about how to use your health insurance or you dont have any. Or perhaps you simply dont like visiting the doctor and prefer an ignorance-is-bliss approach.
Whatever the reason, some sciatica symptoms truly warrant medical attention. In rare cases, delaying medical care could lead to or cause permanent nerve damage.
If you experience any of the following, please see your doctor as soon as possible:
- You have severe pain in your low back and legs
- You experience nerve-related symptoms, such as weakness, numbness, tingling, or electric shock-like pain
- Your pain doesnt improve after 2 weeks
- Your pain gets worse, even when using at-home therapies
- You have loss of bowel and/or bladder control
Easing the extreme pain of sciatica doesnt always require an extreme treatment approach. Cons Of Using Heat To Treat Carpal Tunnel
Too much heat can increase swelling within the carpal canal and lead to increased pressure on the median nerve worsening your symptoms.
Too much heat and the nerve may engorge or get swollen and the pressure in the canal rises.
With either temperature treatment for Carpal Tunnel, it is important to avoid extreme temperatures. Its normal for your skin to look a little pinker after using cold or heat. Before resuming temperature treatment, let your skin return to its normal color and temperature.

Cons Of Ice For Carpal Tunnel
Many people find that, during the cold months, their carpal tunnel becomes exacerbated because too much cold can cause stiffness and pain. This is no different with ice packs. Too much will simply cause slightly different kinds of pain and stiffness in the area. Thats why its important to keep the ice pack on for only 20 minutes.
Ice is not a cure for carpal tunnel, but it does provide temporary relief. Unfortunately, some people come to depend on it, thinking the best they can do is manage their CTS with ice. This is not true, and well get to the best ways to treat carpal tunnel below.
Ice can also be dangerous for those with cold intolerance or a condition that affects how the body perceives pain .
Ice Packs And Ice Baths
Every single day in my office, the topic of ice, heat & carpal tunnelcomes up. Many people swear that applying cold to their hand and wrist relieves carpal tunnel pain. I have no doubt that among all carpal tunnel remedies, patients swear by this one the most.
But there are no studies verifying this. Yet many patients have told me that ice baths when done several times a day can keep the pain to a tolerable level. Also, some patients say contrast baths work even better. Thats when you dunk your hand into cold water for a few minutes, and then into hot water for a few minutes.
In reality, I believe these cold or hot baths are just distractions from pain. The pain comes from a compressed median nerve deep inside your wrist joint: it has nothing to do with skin temperature. Supporting my belief, these same devotees say the pain returns in 10-15 minutes. Coincidentally, thats about how long you can distract the mind.

Heat And Ice For Carpal Tunnel
If youve tried heat therapy and ice therapy separately with no real relief, you may consider using them in an alternating fashion. By switching between heat and ice, you can get the best of both worlds and experience relief, and possibly even speed up the healing process.

Wrist Flex And Extend
The wrist flex and extend exercise provides carpal tunnel pain sufferers with a deep stretch. Individuals should start by extending the arm straight in front with the palms facing down. The affected wrist needs to be bent with the fingers pointing toward the floor. The other hand can increase the stretch gently toward the body. Afterward, the wrist can return to the palm down position.

Cold Therapy For Carpal Tunnel Syndrome
Ice therapy reduces blood flow to the carpal tunnel area when applied to the wrist. This process is called vasoconstriction and can help manage swelling, to allow the median nerve to glide more smoothly through the carpal tunnel. By constricting the blood vessels, cold therapy slows nerve activity, numbing hand pain and making it a good choice to use in combination with other treatments.
When To Not Use Heat Or Cold For Your Lower Back
There are some conditions and situations that should not be treated with heat or cold therapy. For example:
- These therapies must not be used on open wounds, bleeds, or when there is any fluid oozing out of the painful region.
- If you have certain chronic conditions such as multiple sclerosis, poor circulation, spinal cord injuries, diabetes mellitus, and/or rheumatoid arthritis, it is advised to avoid heat therapy. Heat in these conditions may cause excessive burns, skin ulceration, and/or increased inflammation.1
It is also advisable to avoid laying directly on the heat source due to the risk of burns, skin damage, or permanent changes in skin color. A protective barrier such as a cloth or towel may be used between your skin and the source of heat.
In general, many people feel heat therapy works better to relieve their lower back pain compared to cold. Also, taking oral pain-relieving drugs while using these therapies may have an added effect on the overall pain relief.2
