Signs You Might Need Surgery

Most people with sciatica don’t need surgery. But your doctor may suggest it if you have trouble walking, lose control of your bladder or bowels, or your pain gets worse and other treatments don’t help. The best procedure depends on what’s causing your symptoms. The most common one removes the part of your herniated disk that’s pressing on the sciatic nerve. Bonus Tip: Get More Restorative Sleep

The quantity and quality of your sleep can directly affect pain and inflammation in your body. When you sleep, several inflammatory substances peak in your bloodstream and stay active throughout the night. When there is poor sleep quality, these inflammatory agents continue to circulate in the body through the dayâresulting in pain and fatigue.8

A full night of deep, restorative sleep can help decrease inflammatory pain and make you feel refreshed and rejuvenated.

If your sciatica pain gets worse and/or severely disrupts your daily life, check with your doctor about additional pain relief options, which may include medications, manual manipulation, epidural steroid injections, and more.

How Does Electromyography Work And What Can I Expect On The Day Of Testing

The EMG study usually includes two parts: 1) Nerve Conduction Studies and 2) Needle electrode examination .

During the first part of the test , small discs are placed on the skin to record nerve function. Mild electrical stimulation is then applied over the skin to test the ability of the nerves to carry the electrical impulse to the recording discs. You can think of your nerves as being similar to electrical cables. A damaged cable will prevent electricity from running through it and powering the devices it is attached to . In a similar way, damaged nerves will prevent electrical signals from running through and being recorded by the discs attached to your skin.

During the second part of the test , a tiny needle is placed into muscles to directly test the function and health of the muscles. A mild pinprick sensation may be felt when the needle is placed into the muscle. During this part of the testing, no electrical stimulation is delivered through the needle and nothing is injected through the needle tip. You can think of the needle as being similar to a microphone. It is only a recording device. The needle is attached via a cable to a computer which allows your physician to both hear and see what your muscle is doing both at rest and with movement.

What Is The Correct Driving Position

  • Use a back support at the curve of your back. Your knees should be at the same level as your hips.
  • Move the seat close to the steering wheel to support the curve of your back. The seat should be close enough to allow your knees to bend and your feet to reach the pedals.
  • Always wear both the lap and shoulder safety belts. Place the lap belt under your abdomen, as low on your hips as possible and across your upper thighs. Never place the belt above your abdomen. Place the shoulder belt between your breasts. Adjust the shoulder and lap belts as snug as possible.
  • If your vehicle is equipped with an air bag, it is very important to wear your shoulder and lap belts. In addition, always sit back at least 10 inches away from the site where the air bag is stored. On the drivers side, the air bag is located in the steering wheel. When driving, pregnant women should adjust the steering wheel so it is tilted toward the chest and away from the head and abdomen.
  • Reclining The Pigeon Position

The so called pigeon pose is quite common in yoga and opens your hips. There are a few varieties, but one of them is classic. This is one of the first ways to alleviate sciatica pains if you have just been diagnosed.

  • Lie on your back and bring the left leg up to a 90 degree angle.
  • Clasp hands behind your thigh and lift the right leg. Place its ankle on top of the right knee.
  • You will need to hold the pose for around 15 seconds it will stretch your piriformis muscle.
  • Perform the same move with the other leg. You can do a few reps on each side.
  • The pose might be uncomfortable at first. Once you can do it painlessly, you can switch to more complicated alternatives.

Treating Sciatica Pain And Managing Expectations

Many people think that the worse the pain, the more likely something bad is going on. However, this isnt true for sciatica. The body can reabsorb the disc material that is causing symptoms, even for those with severe pain. So, treatment focuses on controlling pain and keeping people as active as possible. If the pain is excruciating, lying down for short periods can help, but prolonged bed rest does not. So, once the pain diminishes, I tell patients to get up and start walking short distances. Since sitting increases pressure on the discs in the lower back, I recommend avoiding prolonged sitting or driving. Many people try treatments like physical therapy, massage, acupuncture, and chiropractic manipulation, but evidence suggests that while these approaches may help typical low back pain, they are less helpful for sciatica. Over-the-counter pain medicines like ibuprofen and naproxen can help. When they dont, I may recommend short-term use of stronger, prescription pain medicines.

Patients often ask about spinal injections where steroid medicine is injected into the affected area. It is worth considering for those with uncontrolled pain or for those with persistent, bothersome symptoms who want to avoid surgery. Injections can provide short-term relief. Like any procedure, it has uncommon risks including more pain, and it doesnt seem to decrease the need for future surgery.

Is The Weight Of Pregnancy The Reason Why So Many Pregnant Women Get Sciatica

How to Relieve SCIATICA and Hip Pain with Two Easy Piriformis Stretches | Dr K & Dr Wil

Its true that sciatica is common in pregnancy but increased weight is not the main reason why pregnant women get sciatica. A better explanation is that certain hormones of pregnancy cause a loosening of their ligaments. Ligaments hold the vertebrae together, protect the disks and keep the spine stable. Loosened ligaments can cause the spine to become unstable and might cause disks to slip, which leads to nerves being pinched and the development of sciatica. The babys weight and position can also add pressure to the nerve.

The good news is there are ways to ease sciatic pain during pregnancy, and the pain goes away after birth. Physical therapy and massage therapy, warm showers, heat, medications and other measures can help. If you are pregnant, be sure to follow good posture techniques during pregnancy to also ease your pain.

Simple Steps To Learning How To Sit With Sciatica Pain

  • Make sure your chair fits your body and is the correct height. You want your knees lower than your hips, so your spine can more easily keep its natural curve.
  • No feet on the coffee table! Say no to the ottoman! It seems like youre offering your body a fully relaxed position, but your legs straight out , youre actually stretching your nerves from head to toe. Muscles like to be stretched, but nerves do not.
  • Plant both your feet on the floor. Avoid crossing your legs or leaning to one side.
  • And there you have it! Thats how to sit with sciatica pain.

While you are in this sitting position, you also need to focus on general body alignment. This means keeping your shoulders in alignment over your hips and your head in alignment with your neck and spine.

In addition, your work equipment can be equally helpful when learning how to sit with sciatica. Your alignment will be easier if you have an entire ergonomic setup that includes proper computer and keyboard placement and arm positioning.

When Your Sciatica Warrants A Visit To Your Doctor

Its important to recognize when at-home therapies arent easing your sciatica. If these treatments dont help you, it may be time to see your personal doctor or spine specialist.

People avoid the doctor for a variety of reasons. Maybe youre unsure about how to use your health insurance or you dont have any. Or perhaps you simply dont like visiting the doctor and prefer an ignorance-is-bliss approach.

Whatever the reason, some sciatica symptoms truly warrant medical attention. In rare cases, delaying medical care could lead to or cause permanent nerve damage.

If you experience any of the following, please see your doctor as soon as possible:

    • You have severe pain in your low back and legs
    • You experience nerve-related symptoms, such as weakness, numbness, tingling, or electric shock-like pain
    • Your pain doesnt improve after 2 weeks
    • Your pain gets worse, even when using at-home therapies
    • You have loss of bowel and/or bladder control

Easing the extreme pain of sciatica doesnt always require an extreme treatment approach. Relieving sciatic nerve pain at home with gentle exercise, ice and heat therapy, proper posture, and medication may go along way to speed your recovery. But the most important thing you can do for your low back and leg pain is to take it seriouslyalways call your doctor if you arent experiencing relief.

What Is The Correct Way To Lift Objects

  • If you must lift objects, do not try to lift objects that are awkward or are heavier than 20 pounds.
  • Before you lift an object, make sure you have firm footing.
  • To pick up an object that is lower than the level of your waist, keep your back straight and bend at your knees and hips. Do not end forward at the waist with your knees straight.
  • Stand with a wide stance close to the object you are trying to pick up, and keep your feet firmly on the ground. Tighten your stomach muscles along with your pelvic floor muscles and lift the object using your leg muscles. Straighten your knees in a steady motion. Dont jerk the object up to your body.
  • Stand completely upright without twisting. Always move your feet forward when lifting an object.
  • If you are lifting an object from a table, slide it to the edge of the table so you can hold it close to your body. Bend your knees so you are close to the object. Use your legs to lift the object and come to a standing position.
  • Avoid lifting heavy objects above waist level.
  • Hold packages close to your body with your arms bent. Keep your stomach muscles tight. Take small steps and go slowly.
  • To lower the object, place your feet as you did to lift. Tighten your stomach muscles, and bend your hips and knees.

Sciatica And Piriformis Syndrome Are Two Conditions Often Presenting With Similar Symptoms Here Is A Guide To The Difference Between The Two As Well As Treatment For Each

The hip is a major contributor to all core work. In fact, when we first learn to sit up as babies, we rely on our hip muscles rather than the muscles of our trunk to do this. Repetitive motion like cycling, running and squatting can irritate the muscles in and around the pelvis, leading to chronic pain or limitation. Sciatica and piriformis syndrome are two conditions often presenting with similar symptoms. Here is a guide to the difference between the two as well as treatment for each.

Improve Your Hip Strength And Stability

Having weak hips can also increase the risk of developing Sciatica.

This is especially true after a hip replacement because your hip muscles are weakened from surgery.

Our best advice? Work on your hip strength well before you have surgery. If you can use the 3 months before surgery to build up strength, you can decrease your risk of developing issues afterward.

Release Your Natural Pain Killers With Massage

      • Manual therapy, a special form of medical massage, can help knead out knots, trigger points, or tension in the muscles.4

There are many forms of massage that can be beneficial, such as myofascial release and Shiatsu .5

These massages may be more effective in relieving sciatica pain from poor posture, muscle strain, or fatigued musculature in the lower spine.

Can Needing A Hip Replacement Cause Sciatica

Yes! The Sciatic nerve can be irritated by needing a total hip replacement. When you have end-stage hip arthritis, you tend to lean forward and avoid extending your hip behind you.

These compensations put a lot of stress on your back. There is also a good chance you are limping and that the hip is getting weaker by the day. The longer this goes on, the more likely it can cause issues in the back and other joints such as the knee or ankle.

The best way to get rid of Sciatica with end-stage hip arthritis and needing a hip replacement is to get it fixed with surgery and have a hip replacement.

Heat Things Up Or Cool Them Down

Hot and cold may be opposites, but both can help keep you comfortable. Cold treatment is usually best for an injury that just happened. After about 72 hours, doctors usually suggest switching to heat. Use an ice pack that’s wrapped in a towel or try a heating pad for about 15-20 minutes at a time. Be careful not to burn your skin.

Compensations After Hip Surgery That Contribute To Sciatica

The second leading cause of Sciatica after a hip replacement is compensating for the surgery, which can irritate the nerve. Most people who reach the age where they have to have a hip replacement also likely have arthritis in their lumbar spine.

When you have surgery, arthritis in the back may not tolerate the changes in walking and muscle tightness, aggravating pre-existing arthritis and Sciatic nerve.

Well cover the other compensations to watch out for after hip surgery in the coming sections.
