“When someone is experiencing anterior knee pain, he or she will often grab or point to the kneecap when describing where the pain is located,” says Dr. Brooks. “Generally speaking, it’s pain around or under the kneecap.”

  • Quadriceps muscles as weakness places more stress on the bone and joint itself to maintain stability; tightness prevents full movement or excursion of the tendons, applying greater pressure to the kneecap

In addition, anterior knee pain can be caused by IT band tightness in runners, cyclists and hikers.

Closed-chain leg strengthening includes exercises where your foot remains in contact with the surface you’re exercising on, such as:

  • Walking
  • A feeling of buckling or giving out
  • Being stuck in one position and unable to release your knee from that position

Losing Weight Can Improve Knee Pain

“Your weight plays a major role in knee pain,” says Bush-Joseph. “If you walked around all day with a backpack that had a 10-pound weight in it, you would feel how achy your back, hips and knees are at the end of the day. That shows you the impact extra weight can have on your joints.”

With each step people take, two to four times their body weight is transmitted through the knee joint, according to Bush-Joseph. Thus, the more you weigh, the harder the impact is on your knee joint.

However, people who are overweight and have arthritic knee pain can lessen the impact and ultimately, relieve knee pain by losing weight. In fact, people with arthritic knees lose about 20 percent of their pain with every 10 pounds of weight loss.

“If you are 20 pounds overweight and you have arthritic knee pain, almost half of your pain will go away by losing 20 pounds,” says Bush-Joseph. Of course, losing 20 pounds isn’t easy. But, if people are able to lose even 10 pounds and add in some stretching and flexibility training, they’ll experience significantly less pain, according to Bush-Joseph.

In the days after knee replacement surgery or another type of injury, it is normal to expect a little pain at night. A few nights of lost sleep are not anything to worry about.

However, there are times when you should see a doctor for knee pain at night. Schedule a regular appointment if you experience the following symptoms:

  • Deformed appearance of the joint
  • Pain that makes weight-bearing impossible

Proper shoes are particularly important during exercise. “If you are taking up running as a newbie or starting a new form of aerobic exercise, getting professionally fitted from someone at a running or sporting goods store can help with knee problems and will certainly lower your incidence of having overuse problems due to footwear,” says Bush-Joseph.

People who do not like to stretch before a workout can still protect their knees by slowly ramping up to top speed rather than jumping full speed into their workout.

Perhaps surprisingly, its not just vigorous movements that can produce pain in and around the knee, but also everyday happenings that require sitting for long periods with knees bent, like watching a movie or riding on an airplane.

Common activities that trigger knee pain include climbing stairs, running and certain sports that involve quick changes of direction, such as basketball, football and soccer, says Yair Kissin, M.D., vice chair of the Department of Orthopedic Surgery at Hackensack University Medical Center. A lot of people, regardless of age, want to maintain their high levels of activity, which is great. But sometimes they hurt themselves that way.

What Causes Knee Pain At Night

Our knees are complex joints responsible for bearing the weight of our bodies as it travels to our feet. They literally help us move through the world smoothly, connecting four bonesthe femur, the tibia, the fibula, and the patellaand two major muscles groups .

Connecting and stabilizing these bones and muscles are ligaments and tendons, including:

  • The anterior cruciate ligament : Located on the front of the knee, the ACL prevents the femur from moving backwards onto the tibia
  • The posterior cruciate ligament : The PCL keeps the femur nestled back where it belongs
  • Medial ligaments: Stabilize the inside of the knee
  • Lateral ligaments: Stabilizs the outside of the knee

In addition to this, articular cartilage lines the patella to smooth movement, as does the meniscus tissue that allows the femur and tibia to glide together. Bursa are fluid-filled sacs inside the joint. They further cushion movement, preventing painful bone against bone rubbing.

Any part of this structure can develop painful conditions or suffer from issues that could cause knee pain at night. In general, the five main causes of knee pain at night are:

    Medication can deliver strong relief for your knee pain, but you must be mindful of the possible side effects and follow directions.

    There is a variety of medication available for knee pain, including both oral and injectable medication, depending on the source of your pain. But most medication used by people with knee pain falls into one of two categories: anti-inflammatories and pain relievers, also known as analgesics.

    Over-The-Counter Medication for Knee Pain

    Most medication that falls in the anti-inflammatory category also has analgesic effects, says William Bargar, MD, director of the Joint Replacement Center with Sutter General Hospital in Sacramento, Calif., and a spokesman for the American Academy of Orthopaedic Surgeons.

    The main over-the-counter drugs are acetaminophen and non-steroidal anti-inflammatory drugs , including aspirin , ibuprofen , and naproxen . These can help with simple sprains or even arthritis.

    Dr. Bargar points out that even though knee pain-relieving medication is available without a prescription, you should use care when taking it. Be sure to follow the instructions for any medication and read the warnings.

    Bargar also advises people with knee pain who are taking large doses of ibuprofen regularly to have blood tests every four months to check for kidney toxicity and anemia.

    How To Use Pain Medications Properly

    There are two types of over-the-counter pain medications that can be used for osteoarthritis. Acetaminophen is a pain reliever but not an anti-inflammatory. It may help with mild knee pain.

    NSAIDs can be more effective because they both relieve pain and reduce inflammation. However, they come with potential side effects and risks. NSAIDs can irritate the lining of the stomach, which may lead to an ulcer or other stomach problems. They also can impair kidney function. Some NSAIDs can increase blood pressure. And theyve been linked to an increased risk for heart disease.

    Because of the risks, Dr. Day cautions against using NSAIDs regularly over long periods of time. Instead, she uses NSAIDs for her patients in two ways. First, people who have a flare-up of pain can take them regularly for three to five days and then stop. Second, they can be used over the long term, but only occasionally, maybe a couple of times a week as needed.

    If youre taking NSAIDs several times a day for long periods of time, Dr. Day advises reducing their use by maximizing the other treatment strategies. She also suggests trying a topical NSAID, such as diclofenac , which has fewer potential side effects.

    Opioid pain relievers are discouraged for long-term treatment of chronic knee pain. The milder narcotic tramadol might be appropriate for occasional use in some people, says Dr. Day.

    A doctor should be contacted if:

    • The knee is severely swollen or has a pronounced abnormality
    • The knee cannot fully straighten or fully bend
    • The knee is severely painful
    • The person cannot bear weight on the knee, or feels as if the knee is going to “give out”
    • The skin over the knee turns hot or red
    • The person has a fever of 100.4° F or higher
    • Knee swelling has been present for 3 days or longer

    A doctor will examine the patient’s knee and ask the patient several questions. The physical examination and patient interview may provide enough information to make an accurate diagnosis. If more information is needed, the doctor may recommend medical imaging, such as an x-ray, or removing fluid from the knee using an in-office procedure called aspiration.

    Leg swelling may be a cause for concernContact a doctor if the swelling affects the leg, not just the knee. Leg swelling can be a sign of a serious health problem.,

    Seek immediate care if the leg swelling occurs suddenly with no known reason, particularly if it is accompanied by symptoms such as leg pain, chest pain, and/or problems breathing, as these may be signs of a life-threatening blood clot.
