What Does Piriformis Syndrome Feel Like

It is unlikely that you will be experiencing back pain as a part of this condition
Remember, although back pain isnt a part of piriformis syndrome, sometimes pain can radiate up into the back, so just because you do have back pain doesnt mean you dont have piriformis syndrome.
Equally, just because you have sciatica alone and no back pain doesnt mean it is automatically piriformis syndrome you can still have back-related sciatica without back pain.
The pain experienced from this condition is usually less severe than the pain associated with a disc bulge or prolapse. Most of my patients with true piriformis syndrome have described their pain as more of an annoyance than debilitating pain.
You may still experience pins and needles or numbness . These symptoms are often eased by lying on your front, which actually often make disc-related and age-related problems in your back feel worse.
Piriformis syndrome can occur in individuals of any age.

The body can reabsorb the disc material that is causing symptoms, even for those with severe pain. So, treatment focuses on controlling pain and keeping people as active as possible. If the pain is excruciating, lying down for short periods can help, but prolonged bed rest does not. So, once the pain diminishes, I tell patients to get up and start walking short distances. Since sitting increases pressure on the discs in the lower back, I recommend avoiding prolonged sitting or driving. Many people try treatments like physical therapy, massage, acupuncture, and chiropractic manipulation, but evidence suggests that while these approaches may help typical low back pain, they are less helpful for sciatica. Over-the-counter pain medicines like ibuprofen and naproxen can help. When they dont, I may recommend short-term use of stronger, prescription pain medicines.
Patients often ask about spinal injections where steroid medicine is injected into the affected area. It is worth considering for those with uncontrolled pain or for those with persistent, bothersome symptoms who want to avoid surgery. Injections can provide short-term relief. Like any procedure, it has uncommon risks including more pain, and it doesnt seem to decrease the need for future surgery.
What Are Common Causes Of Sciatica
While sciatica is most commonly a result of a lumbar disc herniation directly pressing on the nerve, any cause of irritation or inflammation of the sciatic nerve can produce the symptoms of sciatica. This irritation of nerves as a result of an abnormal intervertebral disc is referred to as radiculopathy. Aside from a pinched nerve from a disc, other causes of sciatica include
- irritation of the nerve from adjacent bone, tumors, muscle, internal bleeding, infections in or around the lumbar spine, spondylolisthesis, injury, spinal stenosis, cauda equina syndrome, osteophyte, and other causes.
- Sometimes sciatica can occur because of irritation of the sciatic nerve during pregnancy.

What Is The Efficacy Of Conservative Treatments For Sciatica
Conservative treatment for sciatica is primarily aimed at pain reduction, either by analgesics or by reducing pressure on the nerve root. A recent systematic review found that conservative treatments do not clearly improve the natural course of sciatica in most patients or reduce symptoms. Adequately informing patients about the causes and expected prognosis may be an important part of the management strategy. However, educating patients about sciatica has not been specifically investigated in randomised controlled trials.
