10 Questions about pregabalin (LYRICA) for pain: uses, dosages, and risks

There is no generally accepted ‘stepwise’ approach to treating neuropathic pain. There is an array of drugs with limited evidence for differences in efficacy, and often with troublesome adverse effects. Drug therapy is best used as part of a multifaceted, multidisciplinary, active self-management approach to the physical, psychological, social and vocational impacts of neuropathic pain.5 Assess the nature of the pain experience and inform people of realistic outcomes with treatment.6,7 The primary goal in most cases is to make the pain tolerable not usually to eliminate the pain.6 Aim for medium-term drug therapy with a drug holiday after 6 months. Patients who relapse during a drug holiday can resume treatment.

Guidelines recommend starting drug therapy for neuropathic pain with a TCA or an antiepileptic agent .6-12 The dose of the drugs can be escalated at weekly intervals if tolerated, but it may take several weeks to achieve clinical efficacy.

What To Expect With Pregabalin

Pregabalin is prescribed for people whose nerve pain has not responded to other medicines. Nerve pain medicines do not work for everyone and often they do not relieve pain completely when they do work.

Pregabalin helps lessen the pain you are feeling so that you are able to perform your daily activities and have a better quality of life.

How To Save On Lyrica

Lyrica is a brand-name drug, but it has a cheaper generic version. The lowest GoodRx price for the most common version of generic Lyrica is around $16, or 95% off the average retail price of $381.59.

Lyrica comes only as a capsule, so you cant split it in half, which is a common way for patients to save on some medications. But know this: 75 mg, 150 mg, and 300 mg capsules are roughly the same price.

These medications main effect is on noradrenaline. These are similar to amitryptiline and nortriptyline. However, they work even better on pain combined with depression and anxiety.

Their side effects are similar to the other anti-depressant medication. These are newer drugs, and are thus usually more expensive. In some countries theyre funded, in other countries theyre not.

How Long Does Lyrica Cause Effects

When you start taking Lyrica, it may take from one to four weeks to start controlling pain and seizures.

However, side effects can start much sooner. You may start feeling side effects from Lyrica after your first dose. The body and brain effects include:

  • Angioedema, or swollen hands and feet
  • Blurry vision
  • Problems focusing
  • Sleepiness

Dizziness and sleepiness typically go away after a few weeks, while the other side effects are more likely to continue as long as you take Lyrica.

Animal Toxicology And/or Pharmacology

Dermatopathy

Skin lesions ranging from erythema to necrosis were seen in repeated-dose toxicology studies in both rats and monkeys. The etiology of these skin lesions is unknown. At the maximum recommended human dose of 600 mg/day, there is a 2-fold safety margin for the dermatological lesions. The more severe dermatopathies involving necrosis were associated with pregabalin exposures of approximately 3 to 8 times those achieved in humans given the MRD. No increase in incidence of skin lesions was observed in clinical studies.

Ocular Lesions

Ocular lesions were observed in two lifetime carcinogenicity studies in Wistar rats. These findings were observed at plasma pregabalin exposures greater than or equal to 2 times those achieved in humans given the maximum recommended dose of 600 mg/day. A no-effect dose for ocular lesions was not established. Similar lesions were not observed in lifetime carcinogenicity studies in two strains of mice or in monkeys treated for 1 year.

Lyrica For Nerve Pain After Shingles

Lyrica is FDA-approved to treat nerve pain from shingles.

Shingles is a viral infection thats caused by the same virus that causes chickenpox . Anyone whos had chickenpox is at risk for developing shingles at some point in their life.

Your risk for shingles may increase as you age, as shingles is most common after age 50 years. But shingles can occur in someone of any age whos had chickenpox.

Some common symptoms of shingles include:

  • a rash resembling chickenpox but only affecting certain areas, such as the chest, abdomen , back, and around the waist
  • fluid-filled blisters as part of the rash
  • constant burning, dull pain, or a sharp, stabbing pain that comes and goes

Effectiveness for nerve pain after shingles

Several clinical trials have found Lyrica to be effective for treating pain after shingles in adults.

In one 13-week trial, people rated their pain on an 11-point scale before starting treatment. To be eligible for the trial, people had to have had shingles at least 3 months before and have a pain score of at least 4. They were also allowed to take acetaminophen for pain, in addition to Lyrica or a placebo.

People in the study took either Lyrica or a placebo twice daily. Researchers wanted to see how effective each treatment was at reducing peoples pain scores. At the end of the study:

  • about 20% to 35% of people who took Lyrica had their pain score reduced by at least 50%
  • about 10% of people who took a placebo had their pain score reduced by at least 50%

Pregabalin For Chronic Neuropathic Pain In Adults

LYRICA/PREGABALIN | CAN IT TREAT YOUR PAIN?

Bottom line

Moderate-quality evidence shows that oral pregabalin at doses of 300 mg or 600 mg daily has an important effect on pain in some people with moderate or severe neuropathic pain after shingles, or due to diabetes. Low-quality evidence suggests that oral pregabalin is effective after trauma due to stroke or spinal cord injury. Pregabalin appears not to be effective in neuropathic pain associated with HIV. Very limited evidence is available for neuropathic back pain, neuropathic cancer pain, and some other forms of neuropathic pain.

Background

Neuropathic pain comes from damage to the nervous system. It is different from pain messages that are carried along healthy nerves from damaged tissue . Neuropathic pain is often treated by different medicines from those used for pain from damaged tissue, which we often think of as painkillers. Medicines that are sometimes used to treat depression or epilepsy can be effective in some people with neuropathic pain. One of these is pregabalin. Our definition of a good result was a high level of pain relief and ability to keep taking the medicine without side effects making people stop.

Study characteristics

Key results

Pregabalin is helpful for some people with chronic neuropathic pain. It is not possible to know beforehand who will benefit and who will not. Current knowledge suggests that a short course of treatment is the best way of telling.

Pregabalin May Interact With Other Medications

Pregabalin oral capsule can interact with other medications, vitamins, or herbs you may be taking. An interaction is when a substance changes the way a drug works. This can be harmful or prevent the drug from working well.

To help avoid interactions, your doctor should manage all of your medications carefully. Be sure to tell your doctor about all medications, vitamins, or herbs youre taking. To find out how this drug might interact with something else youre taking, talk with your doctor or pharmacist.

Examples of drugs that can cause interactions with pregabalin are listed below.

You’re Gaining Weight But Haven’t Changed Your Diet

This is the Lyrica side effect people are most concerned about, says Sackheim. Again, scientists dont know exactly why Lyrica can cause weight gain, but any time you take a medication, the ingredients can interact with your bodys own chemicals in different ways. I have seen people gain weight but then when they stop the medication, it does get better, she says. It may be possible to simply lower the dose, and if the weight gain is that much of an issue for you, your doctor may prescribe a different medication.

Grogginess Is Basically Your Middle Name

This is the most common Lyrica side effect, says Sackheim. Every anticonvulsant has a groggy effect on us, she explains. Thats because the meds decrease our neuron’s ability to be excited, which can make us feel tired. I usually start people on the medication at night for that reason, she says. If you take it and feel drugged or wake up still groggy, thats not normal and you should talk to your doctor.

What To Do If You Think Lyrica Is Causing Side Effects

Everything

If youve been affected by Lyrica and are suffering side effects, you should first speak to your doctor. Then, talk to an attorney who handles bad drug lawsuits. He or she can advise you as to possible courses of legal action and determine whether you can recover damages.

What Is Known About This Drug

Pregabalin is a structural analogue of the neurotransmitter gamma-aminobutyric acid . It appears to have a similar spectrum of benefits and harms to that of other adjuvant analgesics used to treat neuropathic pain. It is clinically superior to placebo, and indirect comparisons suggest it is non-inferior for efficacy and safety to amitriptyline and gabapentin.

Can You Take Lyrica Long

You should take Lyrica every day as prescribed by your doctor. Many people will take Lyrica long-term to help control their pain or seizures.

Dont stop taking Lyrica without talking to your doctor first. If you need to stop taking Lyrica, your doctor will slowly discontinue it over a period of time, usually at least one week.

Abruptly stopping Lyrica may lead to side effects such as headaches, diarrhea, nausea, anxiety, problems with sleep, and increased sweating. If you take Lyrica for epilepsy and suddenly stop your medicine, your seizures may occur more often.

This is not all the information you need to know about Lyrica for safe and effective use. Review the full Lyrica information here, and discuss this information and any questions with your doctor or other health care provider.

Why Is Lyrica A Controlled Drug

Both Lyrica and Lyrica CR are classified as schedule V controlled substances because there have been reports of euphoria, increased happiness, an excited state, calmness, and a “high” similar to marijuana with use of pregabalin.

In general, medications in schedule V are considered to have a low potential for abuse compared to medicines in other schedules.

Millions Take Gabapentin For Pain But Theres Scant Evidence It Works

How Long Should I Take Gabapentin For Nerve Pain | Gabapentin Nerve Pain Dosage

There is very little data to justify how these drugs are being used and why they should be in the top 10 in sales, a researcher said.

  • Read in app

One of the most widely prescribed prescription drugs, gabapentin, is being taken by millions of patients despite little or no evidence that it can relieve their pain.

In 2006, I wrote about gabapentin after discovering accidentally that it could counter hot flashes.

The drug was initially approved 25 years ago to treat seizure disorders, but it is now commonly prescribed off-label to treat all kinds of pain, acute and chronic, in addition to hot flashes, chronic cough and a host of other medical problems.

The F.D.A. approves a drug for specific uses and doses if the company demonstrates it is safe and effective for its intended uses, and its benefits outweigh any potential risks. Off-label means that a medical provider can legally prescribe any drug that has been approved by the Food and Drug Administration for any condition, not just the ones for which it was approved. This can leave patients at the mercy of what their doctors think is helpful.

Thus, it can become a patients job to try to determine whether a medication prescribed off-label is both safe and effective for their particular condition. This is no easy task even for well-educated doctors, let alone for desperate patients in pain.

But Dr. Goodman cautioned against going off the drug cold turkey because there can be unpleasant withdrawal symptoms.

What Should I Do If I Forget A Dose

If you are taking the capsules or the oral solution and forget to take a dose and remember a few hours later, take the missed dose as soon as you remember it. However, if it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.

If you are taking the extended-release tablets and forget to take a dose after your evening meal, take the missed dose before bedtime after a snack. If you miss taking the dose prior to bedtime, take your dose the following day after breakfast. If you miss taking the dose after breakfast, take your dose at the usual time after an evening meal and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.

Refractory Severe Neuropathic Pain

Assistance from a multidisciplinary pain service may be required for refractory, severe neuropathic pain, as treatment options are complex.5 The TGA has not approved some of the drugs recommended in guidelines for a neuropathic pain indication and most are not subsidised by the PBS for neuropathic pain.6-12Read additional information.

Tramadol can be considered as a third-line treatment for refractory neuropathic pain.6-12 However, other opioids are not recommended without further pain-medicine specialist input due to the problems with tolerance and dependence.6-12 There is limited evidence for use of selective serotonin reuptake inhibitors or serotonin-noradrenaline reuptake inhibitors in the treatment of neuropathic pain, and further research is required to establish their role.13

Faq: How Often Can You Take Lyrica

Does Lyrica start working the first time you take it?

  • Lyrica generally takes at least a week to start working and can take considerably longer, though some people see a difference earlier than others. According to the Lyrica website, dosages typically start out at 150 mg/day and patients can work up to 300 mg/day over a week or two.

Pregnancy And Fertility Concerns

People who are pregnant or plan to become pregnant should notify their healthcare provider before starting the drug. No adequate, well-controlled studies have been conducted in pregnant humans, and use is only appropriate when the potential benefit to the person who is pregnant outweighs the possible risk to the fetus.

Male fertility may also be impacted by taking Lyrica, so this should be discussed with your healthcare provider. Studies showed that the drug made male animals less fertile. Also, birth defects occurred in the offspring of male animals who were treated with pregabalin.

How Many People Use It

When pregabalin was first listed on the Pharmaceutical Benefits Scheme in 2013 to treat neuropathic pain, the then health minister announced it would help 270,000 Australians. However, between June 2015 and 2016, pregabalins item number was claimed around 650,000 times. This may be because the effects of pregabalin are more predictable than those of its predecessor, gabapentin.

Reason For Pbs Listing

Having a Nerve Block

The PBAC recommended pregabalin for listing on the basis of an indirect comparison with amitriptyline and gabapentin with placebo as the common comparator.21 The PBAC accepted that pregabalin was clinically superior to placebo and clinically no worse than amitriptyline or gabapentin.

The PBAC recommended listing of pregabalin on the basis of acceptable cost-effectiveness compared with placebo in people with uncontrolled neuropathic pain. It noted the clinical need for an alternative to current treatments for neuropathic pain.

A key issue identified by the PBAC was economic uncertainty from the potential for pregabalin use outside the restriction. They considered that it was essential that the drug utilisation subcommittee review usage 12 months after PBS listing.
