U of A researcher recommends exercise over painkillers for treating the painful nerve condition.

Although 85 per cent of people will experience back pain in their lives, only two to 10 per cent will suffer from true sciatica, according to a U of A master’s student in rehabilitation medicine.

While sciatica is one of the most common forms of back pain, people often assume they have it when they don’t, says U of A physical therapy researcher Greg Kawchuk.

That’s because the condition-named for the sciatic nerve running down the spine and through the leg-is a general term for many different situations. It can come on suddenly in an acute, disabling way, resolving after a couple of months, or it can fluctuate over time, he said.

Since the pain tends to radiate, it can be felt in a number of places, from the lower back to the buttocks, groin and down the back or front of the leg. It usually results from compression of the nerve roots as they exit the spine, which is caused when jelly-like cushions between vertebrae herniate, said Kawchuk.

“Sometimes the jelly comes out and it will either push on the nerve root or create inflammation that bothers the nerve. What’s nice is that we know that in the majority of patients, it usually resolves and goes away over time-just not that quickly.”

It’s also possible to have sciatica without back pain, he said, just as it is possible to have symptoms of sciatica without involvement of the back.

What Tests Do Health Care Professionals Use To Diagnose Sciatica

Sciatica is a clinical diagnosis. In other words, the health care professional will be able to make the diagnosis based on the patient’s medical history, a physical examination, and description of his or her symptoms. If the patient has had sciatica for only a brief time and has no sign of any other diseases, no lab studies or X-ray films may be needed.

  • If the pain has not improved after several weeks, CT or MRI scans of the spine may be ordered.
  • If the patient has a history of cancer, HIV infection, IV drug use, or has been taking steroids over a period a time, the doctor may want to evaluate plain X-ray films of the back or a bone scan.
  • Occasionally, laboratory studies may be helpful. A CBC may suggest infection, anemia due to certain cancers, or other unusual causes of sciatica. Elevated sedimentation rate may suggest inflammation somewhere in the body. Urinalysis can suggest a kidney stone if there is blood in the urine, or infection, if there are bacteria and pus in the urine.

Sciatica Exercises To Do At Home

When coming into our office isnt possible, or you need sciatica pain relief immediately, make use of these sciatica exercises to do at home. The only equipment these at-home sciatica exercises require is a foam roller and a lacrosse ball.

  • Glute Foam Roller: You can do this at home stretch with a simple foam roller. As youll see in the video above, Ashley is using the foam roller to roll over her left glute with her left leg extended. You want to focus on one glute muscle at a time so you can use the force of your full body weight to press down onto the foam roller. Spend 30 seconds to a minute on one side and then switch to your other side.
  • Piriformis Stretch: As you can see in the video, Jane pulls her left knee toward her opposite shoulder in order to release tension in the piriformis muscle. If the piriformis muscle is tight, it can put pressure on the sciatic nerve resulting in sciatica pain. Spend 15-30 seconds per side on this stretch but be sure to avoid the bad stretch posture that Jane demonstrates. Its important to keep the hips down on the ground and pull the knee to stretch the piriformis and avoid stretching the back.

    The Indirect Results Of Nerve Compression

    Another reason you may be feeling pain in one of your hips is that when you have a pinched nerve in your back that affects one side of your lower body, you tend to favor that side. The pain may alter the way you move, which can place added stress on your already hard-working hips.

    You probably cant. The symptoms just have too many possible causes. Even when the symptoms seem obviously neurological, they routinely arent. Its just not possible to identify nerve trouble by the symptoms.

    Radiculopathy vs. radicular The term radiculopathy refers to a spinal nerve root in trouble and all of its symptoms: pain, stiffness, weakness, numbness, tingling, and other weird sensations. Radicular pain refers only to the pain .

In a 2012 study of more than 500 patients with pain radiating to the legs, no single symptom, or cluster of symptoms, was clearly linked to actual nerve root pathology.12 Careful histories and exams were done on all of these people to find the true radiculopathy cases, and they just didnt line up with the symptoms. The closest match was just:

    But even that was still misleading: not every patient with sciatica has that symptom, and some with that symptom did not have radiculopathy. A cluster of descriptions also correctly identified quite a few cases, but still misclassified too many. This was the best batch:

    • pain below the knee
    • leg pain worse than back pain
    • numbness or pins and needles in the leg

    Some kinds of symptoms do suggest neuropathy more strongly:

    How Does Sciatica Pain Feel

    Typical sciatica pain radiates from your back through your buttocks and down your leg. The pain can be intermittent or constant and occurs most commonly in the back of your leg however, it may be present in other areas of the leg as well. Burning, numbness, and tingling are also common types of sciatica pain.

    Sciatica leg pain can occur on either side, depending on where the pressure on the nerve is located. It very rarely occurs on both sides.

    How Long Should I Try Self

    Every person with sciatic pain is different. The type of pain can be different, the intensity of pain is different and the cause of the pain can be different. In some patients, a more aggressive treatment may be tried first. However, generally speaking, if a six-week trial of conservative, self-care treatments like ice, heat, stretching, over-the-counter medicines has not provided relief, its time to return to a healthcare professional and try other treatment options.

    When Is Surgery Considered

    Spinal surgery is usually not recommended unless you have not improved with other treatment methods such as stretching and medication, your pain is worsening, you have severe weakness in the muscles in your lower extremities or you have lost bladder or bowel control.

    How soon surgery would be considered depends on the cause of your sciatica. Surgery is typically considered within a year of ongoing symptoms. Pain that is severe and unrelenting and is preventing you from standing or working and youve been admitted to a hospital would require more aggressive treatment and a shorter timeline to surgery. Loss of bladder or bowel control could require emergency surgery if determined to be cauda equine syndrome.

    The goal of spinal surgery for sciatic pain is to remove the pressure on the nerves that are being pinched and to make sure the spine is stable.

    Microdiscectomy: This is a minimally invasive procedure used to remove fragments of a herniated disk that are pressing on a nerve.

    • Laminectomy: In this procedure, the lamina that is causing pressure on the sciatic nerve is removed.

    Fans of fancy footwear: if you have sciatica and just cannot seem to shake the pain, your high heels may be causing your discomfort. High heels change your distribution of weight, putting pressure on the front of your foot and causing you to push your hips forward.

    How To Manage Sciatic Pain

    For many people, sciatica responds well to self-care. Rest for a couple of days after a flare-up begins, but dont wait too long before resuming activity. Long periods of inactivity will actually make your symptoms worse.

    Applying hot or cold packs to your lower back may provide temporary relief. You can also try these six stretches to help relieve sciatic pain.

    Over-the-counter medication, like aspirin or ibuprofen , may help reduce inflammation, swelling, and alleviate some of your pain.

    If your symptoms are severe and home remedies arent reducing your pain, or if your pain is getting worse, see your doctor. They might prescribe medications to relieve your symptoms, such as:

    Your doctor may also suggest that you attend physical therapy after your symptoms improve. Physical therapy can help prevent future flare-ups by strengthening your core and back muscles.

    You doctor might also suggest steroid injections. When injected into the area surrounding the affected nerve, steroids can reduce inflammation and pressure on the nerve. You can only receive a limited number of steroid injections, though, since theres a risk of severe side effects.

    Surgery may be recommended as a last resort if your pain hasnt responded to other treatments. It may also be an option if your sciatica is causing loss of bowel or bladder control.
