ABSTRACT:Pain management is complicated and is becoming more difficult, especially as pain medications are being more strictly controlled. An increasingly popular pain management strategy is the use of noncontrolled topical pain management agents, including diclofenac and lidocaine. These agents are not appropriate for managing all types of pain, but they are a good option for select states of chronic pain. It is important for pharmacists to be aware of the available options in order to contribute to the care of patients with chronic pain.
The rate of death due to overdose of pain medications is rising significantly, with an increase of 400% in women and 265% in men over a 10-year period in the United States.1 When the Drug Enforcement Administration announced in August 2014 the rescheduling of hydrocodone/acetaminophen from a Schedule III to a Schedule II drug, the world of primary care and chronic pain management was thrown into a tailspin.2 Before the rule became effective in October 2014, there were reports of fraudulent Vicodin prescriptions being called in to pharmacies. Concerns over documentation of CIIs began to arise in physicians offices, as this change would increase demands on their staff and inconvenience patients.

How Does Ultrasound Therapy Differ From Shockwave Therapy
Ultrasound therapy differs from shockwave therapy in many ways, mostly that ultrasound therapy is designed to increase blood flow and reduce inflammation, whereas shockwave therapy is designed to also increase blood flow, but it does so by intentionally causing trauma to the tissues by way of using a shockwave.
Shockwave therapy starts the healing process when injuries are stuck in the inflammatory phase of healing, it essentially re-aggravates the injury to get the body to heal itself.
Ultrasound Therapy For Chronic Low Back Pain
The evidence from this systematic review is uncertain regarding the effect of therapeutic ultrasound on pain in individuals with chronic non-specific LBP. Whilst there is some evidence that therapeutic ultrasound may have a small effect on improving low back function in the short term compared to placebo, the certainty of evidence is very low. The true effect is likely to be substantially different. There are few high-quality randomised trials, and the available trials were very small. The current evidence does not support the use of therapeutic ultrasound in the management of chronic LBP.
This is an update of a Cochrane Review published in 2014. Chronic non-specific low back pain has become one of the main causes of disability in the adult population around the world. Although therapeutic ultrasound is not recommended in recent clinical guidelines, it is frequently used by physiotherapists in the treatment of chronic LBP.
The objective of this review was to determine the effectiveness of therapeutic ultrasound in the management of chronic non-specific LBP. A secondary objective was to determine the most effective dosage and intensity of therapeutic ultrasound for chronic LBP.
We performed electronic searches in CENTRAL, MEDLINE, Embase, CINAHL, PEDro, Index to Chiropractic Literature, and two trials registers to 7 January 2020. We checked the reference lists of eligible studies and relevant systematic reviews and performed forward citation searching.

Experimental Groups And Surgical Methods In Vivo
80 SD rats were randomly divided into four groups: the control group : autografting without ultrasonic treatment, low-dose group : autografting with low-dose LIPUS treatment, mid-dose group : autografting with mid-dose LIPUS treatment and high-dose group : autografting with high-dose LIPUS treatment. Rats were anesthetized by 40mg.kg1 injection of 2% sodium pentobarbital into abdominal cavity.
All the rats received sciatic nerve autograft reversal transplantation surgery on the right hind legs. A 10-mm-long sciatic nerve was excised and then grafted in the reverse direction between the two nerve stumps using 9-0 Ethilon sutures on either side to manufacture a 10-mm-long sciatic nerve autograft models. Muscle and skin were closed with 3-0 Ethilon sutures. All animals were raised in cages after operation.
5 rats of each group were sacrificed at week 2, 4, 8 and 12 after surgery for electrophysiological evaluation, ultrasonic testing, molecular biological detection and pathology detection.

A therapist performs ultrasound therapy using a transducer or wand. Inside the ultrasound therapy device, theres a small crystal charged to release sound waves at a frequency of 800,000Hz 2,000,000Hz to stimulate the tissue beneath the skin.
However, before the treatment begins, the physician inspects your body for active wounds, burns, or infections. If the soft tissues are seriously inflamed, the therapist may administer topical medication to reduce the swelling. A process referred to as phonophoresis.
If your skin looks clear, theyll apply a gel on the affected body part or the transducer head to expel air between your skin and the machine head. Once every item checks off, the doctor starts moving the ultrasound head slowly in a circular motion around your body.
Since the machine has different settings, depending on your pain, the therapist may change settings as they perform the ultrasound therapy to control the depth of the ultrasound waves entering your body.

Ultrasound used in physiotherapy is different from the ultrasound used for other medical purposes. Medical or diagnostic ultrasound is used to generate images of the internal structure of the body. Weighing Triceps Surae Muscles And Masson Trichrome Staining
Isolated the operational and contralateral side triceps surae muscles and cleaned the surface blood with physiological saline, dried with filter paper, weighed and recorded their weights by electronic balance scale . Specimens were fixed in 4% paraformaldehyde and paraffin embedded after dehydration, then cross-sectional cut into 5m thick slices in the triceps surae muscle belly and stained with Masson trichrome to measure the cross-sectional areas of the muscle fibers. The data was measured by use of Image Pro Plus. The wet weight recovery ratio of gastrocnemius muscle was calculated as the wet weight of gastrocnemius muscle of the operational side divided by that of the nonoperational side.
