Surgery For A Pancoast Tumor

Can My Shoulder Pain Be Cancer?

Prior to surgery, the doctor carefully assesses and stages the cancer. During surgery, the doctor usually removes a portion of the chest wall and part of the lung. The survival rate associated with this procedure is typically 30% to 50% after five years.People with Pancoast tumors that are directly invading the covering of the lung and chest wall should generally undergo surgery, if:

  • The cancer has not spread to distant parts of the body.
  • The patient’s heart and lungs are healthy enough to allow surgery.
  • There is no evidence of extensive enlargement of the lymph nodes.

The patient’s outlook is then dependent on the status of the lymph nodes. Occasionally, people with severe pain who have tumors that can’t be removed may be selectively considered for palliative surgery to reduce pain.

What Is The Treatment For Lung Cancer

Treatment for lung cancer primarily involves surgical removal of cancer, chemotherapy, or radiation therapy, as well as combinations of these treatments. Targeted therapies and immunotherapy treatments are becoming more common, as well. The decision about which treatments will be appropriate for a given individual must take into account the location and extent of the tumor, as well as the overall health status of the patient.

As with other cancers, doctors may prescribe therapy intended to be curative or palliative . Doctors may prescribe more than one type of therapy. In such cases, the therapy that is added to enhance the effects of the primary therapy is referred to as adjuvant therapy. An example of adjuvant therapy is chemotherapy or radiotherapy administered after surgical removal of a tumor in an attempt to kill any tumor cells that remain following surgery.

The surgical procedure chosen depends upon the size and location of the tumor. Surgeons must open the chest wall and may perform a wedge resection of the lung , a lobectomy , or a pneumonectomy . Sometimes lymph nodes in the region of the lungs also are removed . Surgery for lung cancer is a major surgical procedure that requires general anesthesia, hospitalization, and follow-up care for weeks to months. The risks of surgery include complications due to bleeding, infection, and complications of general anesthesia.

Symptoms That May Suggest Lung Cancer

The symptoms of back pain related to lung cancer are diverse and overlap significantly with back pain caused by other conditions. If it involves the spine, it can mimic many of the symptoms of an upper back injury.

Lung cancer related back pain may be generalized like a muscle ache or sharp like a pinched nerve. People with adrenal gland involvement may sometimes complain of “kidney pain” on one side of their back, or describe a feeling like they’ve just been “kidney punched.”

That said, back pain related to lung cancer may have certain telltale signs. Red flags that back pain may be due to lung cancer include:

    • Back pain that is present at rest
    • Back pain that is worst at night
    • Back pain that occurs without any activity
    • Back pain that worsens the longer you lie in bed
    • Back pain that gets worse when you take a deep breath
    • Back pain that doesn’t respond to physical therapy or other medical interventions

Moreover, back pain accompanied by other telltale signs such as a;persistent cough, shortness of breath,;unintentional weight loss, chronic fatigue, or the;coughing up of blood;may further support the likelihood of lung cancer.

It’s important to note, however, that the “typical” symptoms of lung cancer are less likely to be present with lung adenocarcinoma, and currently the most common symptoms are fatigue and shortness of breath with exercise .

Can Lung Cancer Ever Be Responsible

Lung Cancer and Shoulder Pain: Are They Related?

Yesbut, says Alex Little, MD, a thoracic surgeon with a special interest in esophageal and lung cancer, and clinical professor at the University of Arizona.

The only way a lung cancer can cause pain is by invading through the pleural lining of the lung and growing into the chest wall, explains Dr. Little.

This occurs but is an infrequent occurrence. The most common example is the Pancoast tumor at the top of the lung that causes shoulder pain by invading the nerves in that area.

Im sure most back pain is due to etiologies other than lung cancer.

Pain that persists over time despite standard treatment would be consistent with a lung cancer but, again, lung cancer as a cause is unlikely.

Though lung cancer as a cause of back pain at rest, even in a smoker, is not the most likely cause, theres something else that must be considered.

Other cancers can cause back pain. The concerning issue is if the pain persists at rest, no matter how comfortable the patient tries to get.

What would be more reassuring is if chronic back pain can clearly be associated with certain activities or body positions, and goes away when those offending activities or motions are ceased.

The First Sign That Anything Was Wrong Was When He Coughed Up A Load Of Thick Blood

Early diagnosis of mesothelioma is difficult. The first obvious sign of illness may be when an ‘effusion’ occurs. This is an accumulation of fluid which gathers in the pleural space around the lung, , which causes breathlessness. However, other symptoms may occur too. One man with mesothelioma initially complained of chest pain and a dull ache in his shoulder. Another man had backache, a ‘croaky voice’, weight loss and breathlessness. Several tests and biopsies may be necessary to confirm the diagnosis, and this can take a few weeks.

Be Aware Of All The Symptoms

Some people who are diagnosed with lung cancer like Heather may only experience back pain. Other people may have several other symptoms. Some people even have no symptoms.

As well as back pain, shoulder pain and neck pain, other signs and symptoms of lung cancer include:

    • Persistent cough for 3 weeks or more
    • Wheezing
    • Loss of appetite and/or unexplained weight loss
    • Coughing up blood or blood in your phlegm
    • Unexplained fatigue or lack of energy
    • Hoarseness
    • Swelling in the face or neck

    Breathing Changes Or Wheezing

Shortness of breath or becoming easily winded are also possible symptoms of lung cancer. Changes in breathing can occur if lung cancer blocks or narrows an airway, or if fluid from a lung tumor builds up in the chest.

Make a point of noticing when you feel winded or short of breath. If you find it difficult to breathe after climbing stairs or performing tasks you once found easy, dont ignore it.

When your airways become constricted, blocked, or inflamed, your lungs may produce a wheezing or whistling sound when you breathe. This can have multiple causes, some of which are benign and easily treatable.

However, wheezing is also a symptom of lung cancer, which is why it merits your doctors attention. Dont assume that wheezing is caused by asthma or allergies. Have your doctor confirm the cause.

Early Warning Signs Of Lung Cancer

Cancer symptom pains – Lung Cancer

Most lung cancers don’t cause symptoms until the disease has advanced, in part because the lungs have few nerve endings. When lung cancer does cause signs in its early stages, they may vary from person to person but commonly include:

    • A new cough that is persistent or worsens, or a change in an existing chronic cough
    • Cough that produces blood

Less common symptoms of lung cancer may include:

    • Swelling in the face or neck
    • Difficulty swallowing or pain while swallowing
    • Changes in the appearance of fingers, called finger clubbing

Although most of these symptoms are more likely to be caused by something other than lung cancer, it’s important to see a doctor. Discovering lung cancer early may mean more treatment options are available.

    What Causes Lung Cancer

    Exposure to asbestos fibers

Asbestos fibers are silicate fibers that can persist for a lifetime in lung tissue following exposure to asbestos. The workplace was a common source of exposure to asbestos fibers, as asbestos was widely used in the past as both thermal and acoustic insulation. Today, asbestos use is limited or banned in many countries, including the U.S. Both lung cancer and mesothelioma are associated with exposure to asbestos. Cigarette smoking drastically increases the chance of developing asbestos-related lung cancer in workers exposed to asbestos; asbestos workers who do not smoke have a fivefold greater risk of developing lung cancer than nonsmokers, but asbestos workers who smoke have a risk that is 50- to 90-fold greater than nonsmokers.

      Back Or Shoulder Pain

A Pancoast tumor is a type of lung cancer that grows in the upper part of your lung and spreads to your ribs, vertebrae in your spine, nerves, and blood vessels. Because of where these tumors grow, they rarely affect your respiratory system. They’re more likely to make your shoulder blade, upper back, and arm hurt instead.

Which Type Of Doctors Treat Pancoast Tumors

Pancoast tumors are typically treated by a multidisciplinary team that includes thoracic surgeons or surgical oncologists, medical oncologists, and radiation oncologists.

      • Chest X-ray film
      • In the early stages, Pancoast tumors are difficult to detect on chest X-ray films because the top of the lung is located in an area of the body that is difficult to visualize clearly on an X-ray film. Shadows that lie over the lungs make the image on the X-ray film unclear. Many patients end up consulting orthopedic surgeons and neurologists before a definitive diagnosis is made. An apical lordotic view ordered with the chest X-ray may visualize the problem in patients with suspicious symptoms.
      • A chest X-ray film may reveal asymmetry of the tops of the lungs in the form of a small, uniform patch of tissue or pleural thickening on the apex of one lung. The X-ray film may show a thin plaque at the lung apex in the area of the superior sulcus, or the X-ray film may reveal a large mass, depending on the stage when it is first diagnosed.
      • The plain chest X-ray film may show that the tumor has invaded one or more ribs or parts of the vertebrae. Bone destruction of 1-3 of the back ribs may sometimes be visible on the X-ray film.
      • Enlargement of the mediastinum may be visible on the X-ray film.
    • Arteriogram or a venogram : Rarely, the Pancoast tumor involves the artery or the vein under the clavicle; therefore, an arteriogram or a venogram may be helpful.
    • Exams to determine spread

      • Female smokers are 13 times more prone to develop lung cancer.

But non-smokers get lung cancer too risk factors include radon gas and worked-related exposure to carcinogens. Non-smokers who are exposed to second-hand smoke have a 20-30% greater risk for developing lung cancer.

Lung cancer is most common in people over 65. However, younger adults can and do develop lung cancer, but less than 2% of patients are below 35.

      Physical Therapy And Doubts

I went to physical therapy for a couple of months and did all the exercises prescribed. My neck started to feel better but my elbow did not, so I visited an orthopedist who diagnosed me with tennis elbow. By the way, you evidently DO NOT have to play tennis to get tennis elbow since I hadnt picked up a racket since college. He gave me a cortisone shot and sent me to another physical therapist, who sent me to another orthopedist, who sent me to a rheumatologist. All of these visits ended with me being told I was in perfect health other than my elbow trouble.

At this point, I had some doubts set in. My husband started asking me how long I was going to spend going to doctors and physical therapists. Other family and friends started telling me that everyone gets aches and pains as they get older. However, I was still having trouble bending my right elbow and I didnt think this was something I should just learn to live with at 47 years old!
