  • Why Is Voltaren Mainly For Joints And Only Some Joints

When the FDA refers to joints that are amenable to topical treatment, what do they mean? Is an amenable joint friendly and easy-going? No, just accessible: a pain-killing gel is useful only for joints that are not covered by a thick layer of muscle . The medication gets diluted as it penetrates deeper into tissue, and a meaningful amount can only get into joints if they are just under the surface of the skin. The shoulder, hip, and spinal joints are notably quite deep.

For more superficial joints, though, Voltaren® Gel delivers a good dose of medication, while sparing the gastrointestinal tract from the harshness of NSAIDs which are known as gut burners, because many people just cant stomach ibuprofen. A topical drug mostly eliminates the risks associated with digesting the stuff.4

I am definitely not saying Voltaren is completely safe or risk free. No drug is. The drug is still being absorbed, but instead of being a gut burner it can be a skin burner. From the Voltaren® Gel website

The most common adverse reactions reported in Voltaren Gel clinical trials were application site reactions in 7% of treated patients. With all NSAIDs there may be an increased risk of serious cardiovascular thrombotic events, myocardial infarction, and stroke, which can be fatal.

Sounds bad, doesnt it? But those warnings are primarily there in an abundance of legal and medical caution provoked by the problems with oral NSAIDs. For short-term, moderate topical use, I believe the benefits clearly outweigh the minor risks.

    What Is Diclofenac Topical

    Neck Pain

    Diclofenac is a nonsteroidal anti-inflammatory drug .

Diclofenac topical is used to treat joint pain caused by osteoarthritis. This medicine is for use on the hands, wrists, elbows, knees, ankles, or feet. Diclofenac topical may not be effective in treating arthritis pain elsewhere in the body.

Solaraze is used to treat warty overgrowths of skin on sun-exposed areas of the body.

Diclofenac topical may also be used for purposes not listed in this medication guide.

More About Safety And The Trouble With Oral Diclofenac

Long term and/or large oral doses of any of the NSAIDs can be extremely dangerous, even lethal. They nuke your entire system with much more active ingredient than you really need, all of it absorbed through the digestive tract and distributed through your entire circulatory system.

These drugs can and do cause complications at any dose, and are linked to heart attacks and strokes and ulcerations of the GI tract. What a bunch of jerks!

But oral diclofenac is a special kind of awful.

Diclofenac is an extremely popular drug and it is also associated with serious cardiovascular risks. McGettigan et al:19

There is increasing regulatory concern about diclofenac. Diclofenac has no advantage in terms of gastrointestinal safety and it has a clear cardiovascular disadvantage.

This has been in the news quite a bit, and NPR had a hit in 2013 with this headline: Worlds Most Popular Painkiller Raises Heart Attack Risk. And its not wrong, that headline. Its not hype and alarmism there is a real problem.

    Shoulder Exercises And Stretches

Regular exercises and stretches can keep your shoulder strong and flexible. A few things to remember before swinging in to exercises:

    • Stop any exercise if you have more shoulder pain. It may be too soon for you to try.
    • Watch your form. Exercising incorrectly can also cause or worsen shoulder problems.
    • Warm up, even before deep stretching. Light shoulder rolls, gentle movements, or even a warm shower are all ways to warm up your muscles before exercise and stretching.

Pendulum stretch for range of motion

    • Stand and bend at the waist.
    • Let your arm on the injured side hang straight down.
    • Keep your neck relaxed.
    • Move your arm in a circle up to 20 times.
    • Do once or more times in a day.

    Overhead shoulder stretch

    • Sit or stand to do this shoulder stretch.
    • Intertwine your fingers in front of you.
    • Bend your elbows and raise your arms above your head. You can also place your hands on your head or behind it.
    • Gently squeeze your shoulder blades together to move your elbows back.
    • Continue for up to 20 repetitions. Repeat 5 to 10 times a day.

Youll need treatment along with home remedies if you have a serious shoulder injury. See your doctor if you have any of the following:

    • pain: especially if the pain does not get better with rest and medication
    • repeat problems: if you have shoulder pain more than once
    • stiffness: if you cant lift and rotate your arm normally
    • weakness: if your shoulder, arm, or hand is weaker than the uninjured side

Can I Use Diclofenac On My Dog

Examples of common human NSAIDs include ibuprofen, naproxen sodium, diclofenac, etc. Any type of NSAID should never be given to your dog or cat without consultation with your veterinarian. When NSAIDs are ingested in toxic amounts, it can result in severe gastric ulceration and acute kidney failure in dogs and cats.

Is A Topical Nsaid Right For You

Your Eyes Can Fix Shoulder Pain

The American College of Rheumatology and the Centers for Disease Control and Prevention both recommend topical NSAIDs as one of the initial treatments for arthritic pain.

Topical NSAIDs are best for people with only a few painful joints, says Dr. Konya. If you apply the NSAID to several joints you may exceed the recommended dosage and increase the risk of side effects. Patients with arthritis in multiple joints are better off treated with an oral NSAID or acetaminophen unless theres a medical reason they shouldnt take them.

Topical NSAIDs are particularly good for hand, knee or elbow arthritis. Because these joints are close to the surface of the skin they are more suited to topical treatment than deeper joints like the hip or shoulder.

Topical NSAIDs should always be used with caution in people with known cardiovascular or kidney disease, says Dr. Konya. You shouldnt use topical NSAIDs if you have had asthma, hives or other allergic-type reactions after taking aspirin or other NSAIDs. Also, topical NSAIDs shouldnt be used after open heart surgery, such as coronary artery bypass graft surgery.

What Is The Difference Between Topical And Oral Diclofenac

Diclofenac is a nonsteroidal anti-inflammatory drug that is often used to treat arthritis pain. It comes in both topical and oral forms.

The FDA approval of Voltaren Arthritis Pain is specifically for the topical gel form. Voltaren Arthritis Pain does not offer an oral diclofenac form.

Topical NSAIDs, such as Voltaren Arthritis Pain Gel , should be considered prior to the use of oral NSAIDs, according to the American College of Rheumatology Treatment Guidelines. The goal for this ACR guideline is for patients to achieve the lowest systemic exposure, which refers to the amount of NSAID medicine circulating in the bloodstream.

As a topical gel, a smaller amount of diclofenac from Voltaren Arthritis Pain Gel reaches the bloodstream compared with the oral form of diclofenac. In a clinical study, the systemic exposure with Voltaren Arthritis Pain Gel was 6%, or 94% less than the systemic exposure with the oral NSAID diclofenac.

Voltaren Arthritis Pain Gel is applied directly to the joints and penetrates through the skin on its way to the site of arthritis pain. Voltaren Arthritis Pain Gel also gives you greater flexibility in managing your arthritis pain with FDA approval for use up to 21 days, while oral NSAIDs are indicated for use up to 10 days without consulting a doctor.

How To Use Topical Diclofenac Solutions

Diclofenac gel must be prescribed by a medical professional. Consult with a doctor to determine the diclofenac solution that is right for you, and always follow the accompanying medication guide.

Voltaren and Pennsaid should be applied to clean, dry skin . Avoid showering or bathing for at least 30 minutes after applying these medications. Do not apply a bandage or dressing to the treated area, and avoid heat. Try to maintain a regular application schedule.

Be careful to avoid the eyes, nose, and mouth. Rinse with water or a saline solution if the medication gets in your eyes. If your eyes are still irritated after an hour, call poison control.

Avoid applying diclofenac gels and liquids to peeling, infected, swollen or broken skin. Do not apply to open wounds or areas covered with a rash, as doing so may irritate the skin.

Before using diclofenac gel, patch-test by applying the gel in a small area at first. If the skin shows no signs of irritation after 24 hours, the gel should be safe to use.

Store the gel at room temperature and out of the reach of children.

Follow the directions on your dosing card carefully when using diclofenac gel and be sure to apply it to clean, unbroken skin.

    Voltaren Gel: Behold The Awesomeness

Voltaren 1% gel has a huge fan base, rightly so. Voltaren 1% gel works as well as anti-inflammatory pills for hand and knee pain due to arthritis, with way fewer side effects. Lets get this straight. Yep, results from 34 studies were reviewed and the topical anti-inflammatory Voltaren gel alleviated pain in the hands or knees more often than oral NSAIDS .

Although skin reactions are more common with Voltaren gel than with pills, the gel does not carry the risk of gastrointestinal bleed or kidney problems. This is a big deal. Many patients will prefer to go the route of rubbing a gel on the sore joint rather than taking a pill that can irritate the gut and the kidneys.

Currently, topical NSAIDS like Voltaren gel should be considered first line treatment for knee and hand pain due to arthritis. Voltaren gel is often used successfully for foot and ankle pain as well. Voltaren gel has not been well studied for pain in hips, spine and shoulders and you can imagine its harder to rub the gel on larger areas but I imagine for localized pain we may be heading there with Voltaren gel.

Just beware of overdoing it while medicated!
