- Awake bruxism: You clench your jaw and grind your teeth during the day with this condition. Its usually tied to emotional issues. Feeling anxious, stressed or angry can lead to teeth grinding. - Sleep bruxism: You grind your teeth while asleep with this form, which may cause more harm. You may not get the help you need since youre unaware it is happening. Another challenge with sleep bruxism is that people dont realize how strong theyre clenching their jaw and teeth. They can use up to 250 pounds of force, causing jaw pain and teeth problems. Clenching can also lead to headaches.

The Neck Bones Connected To The Jaw Bone
The temporomandibular joints connect your lower jawbone to your skull. Muscles attached to these joints allow you to talk, chew and swallow. Because these joints connect your neck and jaw, muscle tension from your jaw can move down to your neck. This tension can manifest in aches, spasms, tight muscles and reduced flexibility.
To put it more simply, jaw pain can easily become a pain in the neck.
As many as 70% of neck problems can be related to TMJ. Poor neck posture, usually identified by rounded shoulders and your chin being pushed forward, can also cause issues in your jaw.
What causes TMJ?
The causes of specific TMJ disorders arent always clear. Beyond an obvious injury, there can be multiple sources for a problem, including:
- Stress and anxiety
- Misalignment of a disc in the temporomandibular joint

Home Care Options For Tmj And Scm Pain
Stretching: The first time you stretch your SCM, it is helpful to visualize the muscle by watching yourself in the mirror. Facing the mirror, lean your ear towards your shoulder. When you do so, you will see the tight sternocleidomastoid muscle become readily apparent as a band from the clavicle to the ear. Stretch it by holding your head tilted to one side while rotating your chin in an upwards and slightly counter-rotated direction. Remember to keep the stretch mild. It should not generate any pain.
Self-Massage: Massage is defined as manipulation of the soft tissues to reduce tissue stress and pain. A viable, home-care option, massage for TMD may be self-administered by oneself or a loved one. The sensitivity of the temporomandibular joint should guide massage of the area, limiting techniques to those using fingers or hands. Explore the following types of massage:
Trigger Point Therapy – Identify and compress knots within the SCM muscle. Apply pressure for 30 seconds and then release.
Stripping – Anchor one end of the SCM with one hand. Using the other hand stroke along the length of the muscle with easy pressure.
Kneading – Use a repetitive rolling motion along the side of the neck progressing downwards with slow, continuous movement. This technique is optimal for the head and scalp area. Gentle, using fingertips only, work through the hair to massage the sensitive fascia and muscles of the head and jaw.

Many professionals think teeth grinding is often the result of a combination of physical, psychological, and genetic risk factors. And yes thats true, but thats not the whole story.
The most common cause of teeth grinding is related to the harmony between the top jaw and bottom jaw.
Heres what I mean. The top jaw is fixed in place, only the bottom jaw opens and closes. So what that means is the top jaw is like a stationary fence. The bottom jaw has to open and close within that fence. Thats fine except when the two jaws dont match quite well. It creates disharmony and thats the primary cause for bruxism.
For a moment I want you to think of the top teeth and bottom teeth as puzzle pieces. When the top teeth and bottom teeth close together, the teeth fit together, just like a puzzle. But thats where problems can occur.
Its not a matter of the teeth fitting together, the jaw also has to also be comfortable closing in that position. When the jaws dont match quite well, it creates disharmony. The position feels uncomfortable. Thats the primary cause of bruxism.
Because it feels uncomfortable to the muscles, guess what happens? The person feels the need the grind their teeth.

Many people with TMJ disorders also have bruxism . Because TMJ disorders and bruxism share many common symptoms, diagnosing a specific cause for them can be complex.
Whether it occurs when youre awake or asleep, bruxism can tire your jaw and lead to pain. Similarly to TMJ disorders, stress is one of the main causes of bruxism. Teeth grinding and clenching can also be caused by anger, fear, anxiety and a family history of bruxism.
As with TMJ issues, jaw and neck pain are among the main symptoms of bruxism, since those muscles tighten during teeth clenching and grinding. Other signs include:
Teeth Grinding And Chronic Pain
If you wake from what you thought was a restful nights sleep with a sore jaw, a headache, or a neck ache, its likely that you were grinding your teeth and clenching your jaw as you slept. The condition is called bruxism, and it affects about 1 in 10 adults at nighttime. About 15% of adolescents and up to 50% of children suffer from the uncomfortable problem, too.

Some Other Risk Factors
The teeth and jaw position is one of the most common factors of bruxism but its not the only factor at play. Some other risk factors for teeth grinding are:
- Stress. Studies show that anxiety or stress increase the frequency of teeth grinding
- Personality type. People with type-a personalities or who are competitive generally are more prone to bruxism.
- Medications or recreational drugs. Some prescription medications, substances, and recreational drugs may increase bruxism. Psychiatric medications, cigarettes, caffeine, and recreational drugs that are stimulants increase the risk of teeth grinding.
- Age. Young children are prone to bruxism during growth years but that doesnt mean they will be bruxers in adulthood.
- Missing teeth. Numerous extractions of teeth can decrease the stability of the jaw and place additional stress on the remaining teeth which causes the tendency to grind.
- Other Conditions. Bruxism can be a symptoms associated with other medical disorders, such as Parkinsons disease, dementia, gastroesophageal reflux disorder , epilepsy, night terrors, sleep-related disorders such as sleep apnea, and attention-deficit/hyperactivity disorder .
What Causes Clenching Or Grinding
At night whilst sleeping there could be many different or multiple causes:
- Intense concentration: The American Academy of Oral Medicine suggests that concentration itself is sufficient to trigger bruxism.
- Stress, Anger or Anxiety: Emotional stress is believed to be a common trigger for grinding or clenching teeth. This may lead to bruxism during the day and/or at night.
- Allergies or blocked nose: Not being able to breathe through your nose due to allergies or being in a stuffy or dry room may contribute to more mouth breathing, which triggers the autonomic nervous system when sleeping. When this is switched on, muscle activity is under less of your voluntary control, and bruxism may be more likely to occur.
- Smoking: Bruxism is twice as common in smokers than non-smokers.
- Excessive Alcohol or Caffeine: These substances can reduce the quality of your sleep and trigger bruxism.
- Illness: Physical illness and poor nutrition can be a contributor.
- Dehydration: Insufficient fluids throughout the day may worsen bruxism.
- Teeth misalignment: a poor bite or malocclusion has historically been thought to play a role in bruxism but a number of studies have failed to demonstrate the link. That said, poorly designed filings that sit too high may also contribute to grinding.
- Genetic factors: If family members clench or grind then you are more prone to develop the habit as well.

Preventing Neck Pain And Teeth Clenching And Grinding During Sleep
The number one thing you can do to prevent neck pain is to maintain good posture. This means aligning your shoulders with your hips, keeping your back straight , and holding your head up. In addition, you should make sure your computer monitor is at eye level and refrain from carrying heavy shoulder bags with straps.
At night, you can prevent neck pain by sleeping in a position that keeps your body in alignment. The best sleeping position to prevent grinding your teeth during sleep and to prevent neck pain is flat on your back. You can also put a pillow under your knees and a rolled up towel beneath the small of your back if you need additional support. Make certain that your neck is in comfortable alignment with your spine before drifting off, because even sleeping on your back can cause problems if your neck is not supported properly.
It’s also possible to sleep on your side if you want to avoid neck pain. You may need more extra pillows for support. For one, you will want to put a pillow between your knees whether you sleep on your left or right side. You may also want a pillow to support the small of your back. Your pillow should support your neck without lifting it too high. Your arm should not be underneath your head. You can also sleep in a fetal position and keep your neck supported. This can help if you also have back pain.
Other Symptoms Of Tmj Disorder
In addition, TMJ disorder causes your jaw muscles to pull on other parts of your body like the top of your head, your neck, and your shoulders. This can result in muscle aches, chronic headaches, and even inner ear pain. Dizziness, jaw popping, and difficulty opening your mouth are other symptoms of TMJ disorder.

Headache Neck And Back Pain Are Symptoms Of Grinding Teeth
Teeth grinding can be the cause of different symptoms which include facial pain, migraines, headaches, ear-ache, tightness and stiffness in the shoulders, pain and stiffness in the jaw joint, abnormal tooth wear and restless or disrupted sleep.
Although teeth grinding can be caused by stress and anxiety, it often occurs during sleep and is more likely caused by an abnormal bite or missing teeth. However involuntary clenching of the teeth and jaw bracing in reaction to certain stimuli plays a role too. So while were awake we could be clenching jaw in reaction to stress or other factors like the style of work we do, some people clench whilst concentrating or doing physical work like lifting and pushing heavy objects. Whilst sleeping teeth grinding is automatic with rhythmic and sustained jaw muscle contractions. There are certain medications that can cause teeth grinding such as antidepressants or recreational drugs such as cocaine and ecstasy, and disorders such as Parkinsons disease, depression and major anxiety.
